Home care and home health services are a comprehensive approach to delivering both skilled and non-skilled care to individuals in the comfort of their own homes. These services are designed to support patients in maintaining their independence, managing chronic conditions, recovering from illness or surgery, and improving overall health and quality of life.
Skilled Services
Home health care includes a range of skilled clinical services provided by licensed professionals. These services are typically prescribed by a physician and may include:
Nursing Care - Monitoring health status, managing medications, wound care, and providing education on disease management. NOTE: Medication management alone is not a skilled need under Medicare guidelines.
Physical Therapy - Assisting patients in regaining strength, mobility, and functional ability following illness, injury, or surgery.
Occupational Therapy - Helping individuals improve their ability to perform daily activities and promoting adaptations to increase safety and independence at home.
Speech Therapy - Addressing communication disorders, cognitive-linguistic impairments, and swallowing difficulties.
Home Care Services
Home care focuses on assisting with daily living activities and creating a safe, comfortable living environment. These services are typically provided by trained aides or caregivers and may include:
- Personal Care - Assistance with bathing, grooming, dressing, and mobility.
- Homemaking Services - Support with meal preparation, light housekeeping, laundry, and errands.
- Respite Care - Provided to give caregivers and loved ones relief.
- Adult Companionship - Focuses on providing social interaction, emotional support, and meaningful engagement.
