Apply through the online portal or print and send the completed and signed form to your local DSS office, or visit your local DSS office.
Eligibility
Single individuals who are in need of Long-Term Care or other related Medical Assistance, Married individuals or couples who are in need of Long-Term Care or other related Medical Assistance, and Married couples who wish to receive a Resource Assessment.
