RHD's Shared Living Program develops housing and community-based support services for individuals with intellectual and developmental disabilities. These include: - Host Home - Adults with intellectual and developmental disabilities get matched with a shared living provider and move into the provider home. The shared living provider is responsible to provide supports and services that foster independence. - Transition Home - This home is staffed 24/7 and operated like a group home. It is used to house individuals while they are matching with their potential shared living providers.

Individuals required to pay a room and board stipend.

Eligibility

Must be 21 or older. Must be on Medicaid and on the CHOICES waiver.
