You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Home Health can help patients make a good transition from hospital to home, or receive the help and care they need at home. Services include physical and occupational therapy, medication management, wound care, IVs and central lines, personal care, light housekeeping and meal preparation.

Retail pharmacy and a medication disposal site. Services provided are immunizations, medical equipment/supplies, smoking cessation, and pain management. Also sells OTC medications.

Black Hills Urgent Care offers injury and illness care, immunizations and vaccinations, basic physicals and physicals for school, sports, and employment, and occupational medicine. COVID-19 tests are available.

Offers the following services: primary care, birth control, morning-after pill (emergency contraception), breast and chest exams including mammogram referrals if necessary, HIV testing, LGBTQ Services, men's health care, pregnancy testing and services, STD testing/treatment/vaccines, women's health care, gender affirming hormone therapy (GAHT), HPV vaccinations, Hepatitis B vaccinations, and Flu shots. Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Hope Harbor's small, family style program is an alternative option for families seeking help for their teen; providing counseling, education, life skills training, spiritual growth opportunities, and parenting classes. Program is Bible-based, Christ-centered, and interdenominational. Minnesota location is licensed by the State of Minnesota Department of Human Services and South Dakota home operates as a non-accredited private school that provides housing. Also provides individual and family biblical counseling, group teaching, instruction needed to continue academic training, life skills training, development of leisure skills, and opportunities for spiritual growth. Provides workshops for schools and communities dealing with the topic of relational aggression, cyberbullying, human trafficking, social media safety and more.

Community-based counseling center that provide a wide range of social and psychological services, including professional readjustment counseling to eligible Veterans, active duty service members, including National Guard and Reserve components, and their families. Bereavement services for family members is also available. Readjustment counseling is offered to make a successful transition from military to civilian life or after a traumatic event experienced in the military. Individual, group, couples, and family counseling is offered in addition to referral and connection to other VA or community benefits and services.

Individual and family therapy services specializing in eye movement desensitization and reprocessing (EMDR), trauma and abuse focused cognitive behavioral therapy (CBT), critical incident stress management, Thinking for a Change Group, and Love and Logic Parenting. Keeping children safe by providing child safety seats at no cost to families that meet income eligibility requirements to ensure that they are in the best child seat for their height and weight until they are at least 4'9. At 4'9, most children can safely wear a seat belt. Current South Dakota law states that any operator of any passenger vehicle transporting a child under five years of age on the streets and highways of this state shall properly secure the child in a child passenger restraint system, and that the operator of a motor vehicle must make sure that a passenger who is at least five and younger than 18 must be wearing a properly adjusted and fastened safety seat belt system. If the belts don't fit properly, the operator must find another safety belt system to secure the child (SD Law 32-37). This program distributes child seats to income eligible parents and children statewide, by assessing financial need. The communities listed may have local distribution partners.
