A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Offering a wide variety of outpatient physical therapy services including: orthopedics, spine care, sports medicine, headaches, TMJ management, dry needling, post-surgical rehab, aquatic therapy, hand therapy, chronic pain, amputee rehab, women's health, pediatric physical therapy, accidents & injuries, kinesiotaping, custom orthotics, and vestibular rehabilitation. The Health Cooperative is here to partner with patients to stay healthy and meet their health goals. An inter-professional team is available to offer early evaluations of adult health concerns. Also provides personalized self-care education that will assist patients in making healthier choices. This is not a replacement for a primary care doctor. Works alongside patients and their doctors to improve patient health. Team consists of registered nurses, a pharmacist, an occupational therapist, a physical therapist and a dietitian.

Investigates child abuse and neglect cases by working with the State's Attorney, law enforcement, and community resources to determine extent of alleged child abuse or neglect. Other services include adoptions, licensing of foster homes, assessment services, case management, and intensive family services. Provide addictions assessments and mental health therapy to adolescents through adults. Community mental health center offering the following services: Moral Recognition Therapy (MRT) Group for adolescents and adults, Dialectical Behavioral Therapy (DBT), individual counseling, marriage and family counseling, psychotropic medication management, domestic violence evaluations and follow up services, psychiatric evaluations and medication management. Walk-in assessments are available at the Watertown and Sisseton locations. Also offers and telehealth services.

Offers individual, couples and family therapy in both English and Spanish. Services include dialectical behavioral therapy, trauma informed therapy, cognitive behavioral therapy, EMDR therapy and counseling for LGBTQ+, child abuse and child abuse survivor counseling for adults, grief and bereavement counseling, terminal illness counseling, and gender identity counseling.

Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria. Provides help for older adults and people with disabilities.

Individual, couples, or family therapy for the treatment of children, adolescents, adults, and seniors focusing on issues including, but are not limited to anxiety, mood, personality disorders, somatization disorders, psychosexual disorders, schizophrenia, post-traumatic stress (PTSD), abuse issues, eating disorders, dementia, grief and loss, childhood emotional and behavioral disorders, spirituality, stress management, and executive health. Evaluations available include psychiatric, psychological, neuropsychological, forensic, and custody.

Horizon Health - Isabel is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Isabel is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

Provides shelter to homeless people in tiny homes, with a main house where they may do laundry, cook, eat, and socialize. Also provides emergency food to very low income households, delivering to those without transportation.

Support group for women who have witnessed or experienced violence in a relationship. These groups seek to provide support and feedback to women who have been emotionally, sexually, or physically abused in their relationships. Topics of discussion include: the dynamics of domestic violence, crisis issues caused by domestic violence, the impact of traumatic experiences on daily living, staying safe, self-esteem, and healthy relationships.The women's support groups are open and may be joined by anyone at any time. No pre-registration is required to attend and groups are offered free of charge.

South Dakota Parent Connection is a statewide nonprofit organization that helps families, caregivers, and professionals access information, resources, and support for children and youth with disabilities, special health care needs, and developmental concerns. The organization provides free, confidential assistance to help families better understand educational, health care, and community systems. Staff members work alongside families to answer questions, provide information, and connect them with resources that can help meet their unique needs. Services include assistance with:

• Connecting with local, state, and national resources
• Understanding special education services, IEPs, and Section 504 plans
• Navigating disability-related programs and community resources
• Accessing information about health care and support services
• Preparing for school meetings and improving family-school communication
• Planning for life after high school and transition to adulthood
• Finding educational trainings, workshops, and learning opportunities

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Provides car seat installation and safety checks at any Sioux Falls Fire Station from 8:00am - 8:00pm (CT), Monday - Sunday. ​Individuals are encouraged to call 211 to register before arriving at a fire station.

Meals on Wheels Western South Dakota are funding lunch at no charge for seniors (60+) accompanying their grandchildren. ​The Black Hills Area Community Foundation will fund lunch for adults (19-59 years old) accompanying their children.
