Horizon Health - Howard Medical is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Loving School at Sioux Falls Children's Home and Owen School at Black Hills Children's Home serves students ages 4-14, in grades PreK through 8th grade, including children in the Sioux Falls and Black Hills Residential Treatment Program. Specializes in meeting educational needs relating to childhood trauma, attention deficit hyperactivity disorder, reactive attachment disorder, abuse and neglect, anxiety disorder, aggression, and parent-child challenges. Students receive individual and group therapy weekly and have access to medical care and psychiatric services, as needed. A student does not need an Individualized Education Plan (IEP) to be enrolled in Children's Home Society schools. If a student does have an IEP, the IEP is implemented and managed by Loving School staff or Owen School staff. Each student's treatment team meets quarterly to share progress and monitor treatment goals. Occupational Therapy and Speech/Language services are available as prescribed by an IEP.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Center Amenities and Services offered include daily meals and snacks, food/hygiene pantry, access to shower and laundry facilities, social and recreational activities (e.g. basketball and volleyball courts, gaming area, and computer lab), Creative Arts (i.e. on-site recording studio, arts and crafts room), Life Skills Workshops, Homework Help, Tutoring, and an on-site K-12 homeschool option. Additional programs offered include a Street Outreach Team that connects homeless or at-risk youth to basic necessities and Resource Navigators to connect youth and families with vital services ---mentorship, obtaining personal identification documents, pursuing employment opportunities, and/or finding emergency shelter/stable housing.

Provides emergency assistance for those in either Minnehaha/Lincoln County with rent and deposits, Utility bills: Must have a disconnect notice for water, lights, or heating; Food and non-food items, Burial: Clients should begin by choosing a funeral home and meet with them prior to applying for assistance through Minnehaha County. Also provides transportation assistance, dental care, medical care (hospitalization) and Medications. Minnehaha County residents can also receive assistance with Shelter, Gasoline or bus ticket for transients/nonresidents stranded here and needing to get home. Out of town travel assistance is not offered to MC residents. Identification and or birth certificate (if needed for the purposes of obtaining employment/housing). The majority of assistance provided is in the form of a no-interest loan for which a lien is created. The client does not receive any bill from County but is expected to repay the debt, starting in a time frame created by the caseworker. Applicants can choose to walk-in to the office or receive a phone call to complete an interview. Those applying for transportation, medications or ID/BC assistance should come into the office to apply.

Helping people identify long term situations that lead to assistance requests and to walk along side them as they bridge out of those situations. Provides assistance for those in the Whitewood area only (cannot assist individuals from Spearfish or other areas). Services provided might include utility payments on a case by case basis, clothing, food, gas, financial education, training and education assistance, and micro-loans (0% interest) for rent deposit; amount must be repaid. Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria. Partners with Main Gate Counseling to provide care for clients to help them in their recovery. Offers peer and youth support for those suffering from a loved one's addiction or children that need counseling.

A no-cost warm, home-cooked meal served every Monday night (except major holidays) with supervised children's area with activities; diapers and baby wipes available each week; household items (toilet paper, etc.); and personal care items (shampoo, deodorant, toothpaste, etc.).

Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Offers assisted living, designed for seniors who want to live independently but need assistance with some daily tasks. Additional services are also available, including on-site physical, speech and occupational therapy. Professional staff are on duty 24/7 to provide assistance.

Provides food to youth who are identified as homeless or at risk of houseless.

Offers clothing and other household goods for persons who wish to use the store's services. If an income-eligible person or family cannot afford to purchase clothing, the Community Service Worker in that county can provide a voucher that allows the person or family to get the clothing at no charge.

A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.
