Mobile food pantry that may be utilized once a month by families and/or individuals. There are no referrals required to receive this service. Individuals MUST enter the church and bring a valid form of ID. This program does not do delivery and is not an emergency food option.

A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

Clients receive help with depression, anxiety, behavioral problems, adjustment issues, grief and loss, as well as other mental health issues. An adolescent outpatient, time-limited, intensive, and coordinated clinical treatment program with the goal of stabilization with the intention of averting inpatient hospitalization or reducing the length of a hospital stay. Partial Hospital Program (PHP): Serves as a positive alternative when individual counseling does not fully meet the needs of the individual and when inpatient care is not necessary. 5 days per week, 4 hours per day. Intensive Outpatient Program (IOP): The goal of program is to provide support while promoting independence, growth, individuality and self-reliance. Clients are assisted in learning new and more effective coping skills while remaining in their homes and their jobs. Generally 7 - 11 hours of structured programming per week consisting of counseling and education groups.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Serving the paratransit needs of Sioux Falls by working with the City of Sioux Falls, DakotAbilities, and Lifescape. Contracted with DakotAbilities, Lifescape, the Sioux Falls School District and works through the City of Sioux Falls to help provide paratransit services to the Sioux Falls area. ​Accepts ride requests for any transportation needs in the Sioux Empire area and beyond. Call for more information. Accepts Medicaid and provides medical rides in Sioux Falls. Coordinates with all other transit agencies in South Dakota and can facilitate a ride statewide. Offers primary care, mental health care, treatment for post traumatic stress disorder (PTSD), and suicide prevention. Also offers transitional and supportive housing for veterans experiencing homelessness. Gun safety locks are available through the Suicide Prevention Coordinator.

Individual and family counseling for issues involving depression, anxiety, grief and loss, ADHD, and PTSD. Therapist is also fully trained in eye movement desensitization and reprocessing (EMDR).

Therapists, counselors, and case managers provide: Individual and family counseling (e.g., marriage counseling, parenting skills, trauma informed services, EMDR, depression, anxiety, PTSD, anger management, coping skill development, etc.) Addiction and dependency services Evaluations and assessments Social learning programs Systems of Care Prevention services, providing presentations to student and adult groups on a large number of topics and consists of a library that contains over 1,000 items available for check out at the main Lemmon office Offering services via telehealth. A food pantry that consists of non-perishable food items and personal hygiene items.

Paratransit services available to persons with disabilities, and features curb-to-curb service. Lift equipped buses for individuals with special needs. Clients must provide their own wheelchair equipped with foot pedals. Will not do any lifting. Clients provide own assistance. Transportation is provided for pre-school age children daily. Bus from Redfield to Aberdeen 1st Thursday of each month.

The West River Area Health Education Center is committed to expanding the healthcare workforce in South Dakota, as well as offering creative, hands-on and innovative health career curriculums for pre-college level students.

ICAP Emergency Services helps with emergency utilities and rent (Rent assistance is NOT available to those residing in Sioux Falls) for income qualified households. Must meet all requirements. Also offers food pantry vouchers to those who qualify (cannot receive SNAP).

Yankton Thrive offers and supports a variety of events and programs in the community including community advocacy, business and career expo, business resources and seminars, holiday promotions, leadership programs, ribbon cuttings, and information on community child care providers, after school programs, healthcare services, and housing rentals. Provides the food vouchers, prescription expense assistance (medicine only), vision expense assistance (eyeglasses and contacts), and personal care items (laundry soap, cleaning supplies, and hygiene items). Please note, prescription assistance is for those without insurance and it is for one time only! Personal care assistance is once in a calendar year.

Offers access to crisis intervention, emergency resources, and case management support for those living in poverty.
