Provides equine-assisted counseling conducted in an arena with a team including a mental health professional, an equine professional, and one or more horses for individuals, couples, families, or groups. Specializing in Post Traumatic Stress Disorder (PTSD), moral injury, trauma, addiction, grief and loss, abuse, depression, anxiety, and mood disorders. Team building and professional development workshops are also available.

Services provided include prescription pick-up (these are delivered from Freeman daily), diabetes education and support, durable medical equipment (i.e. canes and walkers, bathroom equipment, orthopedic supplies, nebulizers, etc.), immunizations, long-term pharmacy, Medicare open enrollment, and a medication disposal site. Accepts UPS drop off packages for free. Customers can use the My Community Pharmacy to refill medications, set medication reminders, and track refill status.

Home Health can help patients make a good transition from hospital to home, or receive the help and care they need at home. Services include physical and occupational therapy, medication management, wound care, IVs and central lines, personal care, light housekeeping and meal preparation.

Provides a helping hand and necessities once a month to families with children up to the age of 5: diapers, wipes, formula, blankets, personal hygiene products, period supplies and feminine hygiene products, and can provide a car seat one time to eligible low-income families. Mommy's Closet is a safe place for parents or other caregivers to come and problem solve issues surrounding parenting, housing, education, employment, and general finances. Foster parents, grandparents and other caregivers are eligible to receive assistance for children in their care. Mommy's Closet caseworker can provide case management and client support to families to help them: identify barriers to self-sufficiency, assist them to create success plans, provide guidance to families as they stabilize their lives, and provide referrals for additional assistance.

Provides mutual coordination, cooperation, and communication among foster parents, agency representatives, and the community to improve the foster care system and enhance the lives of foster children and foster families. Raises awareness of current issues, increasing community awareness of the foster care program, and the needs for foster families, providing training and continuing education for foster parents in order to improve recruitment, retention, and the foster family effectiveness. Provides information and referral services to organizations that host support group meetings for foster families and connects them to community organizations that can support families.

Caseworkers work with women who are going through an unplanned pregnancy. Support services are available to clients at no-cost. Includes: options counseling, evidence-based decision making, adoption planning, adoption parent selection, parenting preparedness program & courses, before & after birth support, mental health counseling, Medicaid & WIC applications, transportation assistance, case management, job application & resume writing, help with difficult family or partner communication, community & state resource connection, and referrals related to housing, substance use, and domestic violence.

A non-profit, pay-what-you-can cafe. People may donate for the meals by cash, credit or volunteer an hour for their meal. There is a $3.00 minimum to select an item from the menu. If individuals wish to volunteer for their meal, volunteering is at 8:00am or 1:00pm. Information for non-profits wanting to make a referral: Family meal referrals can be requested by local non-profit organizations 48 hours prior, details needed include: family name, contact number, number in household & distribution information. These meals include 3 meals for the entire family. Pick up will be available from 11:00am - 1:00pm, Monday - Friday Fork Real works with individuals needing to complete Community Service hours. Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.
