Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Provides an opportunity to have infant or toddler car seat checked by trained staff. Arrive at appointment with the car seat already installed in vehicle. Staff will check the seat and assist to make any necessary changes to the installation of the seat.

Youth & Family Services (YFS) Rapid City Head Start is a federally funded, no-cost, parent-focused education program that serves low-income children, ages 4 weeks-5 years, and their families. Program goals include increasing social skills and school readiness for young children while increasing the self-sufficiency for families. YFS Rapid City Head Start gives priority to families who meet the household income guidelines or have children with disabilities. NeSoDak - Located on a beautiful Glacial Lake, NeSoDak provides a safe place for youth to build relationships, develop new skills and live in a grace-filled community. NeSoDak specializes in summer youth programs, including 1-2-3 camps (two nights) for elementary youth, half-week and week-long camps for middle school through junior high youth, Servant Journey and adventures camps. Klein Ranch - Klein Ranch is a unique camp that offers youth the opportunity to begin to stretch their boundaries, and abilities, as they ride horses on 2,300 acres of Wild West ranch country. Campers can spend a week exploring the prairie on horseback and participating in other activities, such as hiking the mini badlands, having an overnight campout, playing games and more. Through the camp's small group community, youth strengthen their identity and gain depth in their faith. At Klein Ranch, youth also develop a sense of responsibility for themselves and for others as they care for the animals at the camp. Outlaw Ranch - Located amongst granite outcroppings in the southern Black Hills, Outlaw Ranch is open year round, offering summer youth camps, family camp, year round retreats for youth and adults, as well as hosting business gatherings and family reunions. Outlaw Ranch is known across the country for its top notch family camps, bringing youth and families together.

Investigates child abuse and neglect cases by working with the State's Attorney, law enforcement, and community resources to determine extent of alleged child abuse or neglect. Other services include adoptions, licensing of foster homes, assessment services, case management, and intensive family services.
