Provides personal hygiene items and household cleaning essentials to individuals who are in need. Quantity of items is based on household size and all items requested will be provided if in stock. Assistance may be requested once every 30 days, as needed. Pick up site for items is at the First Congregational Church.

Mobile food pantry that may be utilized once a month by families and/or individuals. There are no referrals required to receive this service. Individuals MUST enter the church and bring a valid form of ID. This program does not do delivery and is not an emergency food option.

Individualized treatment including psychiatric and psychological evaluations, individual, couple, and/or family therapy. ICAP Housing Services provides the following programs: Rapid Rehousing and Homeless Prevention Programs: aims to help two specific groups - persons who are at great risk of homelessness and those who have become homeless. Services that can be provided include case management, security deposit and rent assistance. These services are aimed at helping participants to retain their housing, therefore avoiding homelessness, or to re-enter appropriate permanent housing as quickly as possible. Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Homeless Rent and Deposit: Helping homeless households get into housing. Must provide steady and reliable source of income. Applicant must agree to case management services.

Proceeds from the Cornerstone Thrift Store assist the Cornerstone Women and Children's Home. The Store recycles donated clothing, furniture (including mattresses when available), household goods, and other items by offering them for purchase.

The Extended Care and Rehab Service Line includes the following services: Caregiver support group, spinal cord injury / disorder program, full suite of therapies, chiropractic care, integrative health services, adaptive sports, low vision and visually impaired services, inpatient community living center, pain management, whole health services, community living center, hospice/palliative Care, polytrauma/traumatic brain injury (TBI), geriatrics & extended care (GEC), medical foster home, and home health care.

Independent Living Skills: Provides hands-on training in daily-living skills including cooking, budgeting, organizing, and more. Information and referral: Connects people with resources which can provide needed information to assist individuals to live independently. Peer counseling: Provides opportunities for people with disabilities to engage with others in small or large groups or on an individual basis. Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Provides subsidized housing assistance to low-income families.

Provides Housing Choice Voucher (Section 8) tenant based rental assistance to families and individuals who qualify. Operates subsidized apartments, supportive apartments, and market rate apartments. Provides security deposit assistance to eligible applicants when grant funds are available.

Provides physical therapy, occupational therapy and speech therapy for individuals in need of rehabilitation. Please check with the location to see if provided. Must have a referral from a medical provider prior to evaluation and treatment depending on insurance requirements. The outpatient center is equipped to treat a wide variety of patients and diagnoses. There is also an occupational therapy driver's evaluation and training program available at the West 18th Street location.

Matches high school students with adult mentors. They are matched through graduation. Mentors provide additional support as youth navigate through high school. Mentors and students meet one-on-one for four hours a month. Group activities, events, and service projects are held monthly for the matches to take part in. Youth are also eligible to apply for scholarships provided specifically for youth participating in the program.

To provide assistance through education, training, awareness and support to enable Head Start children and families to identify and remove obstacles that keep them from attaining their fullest potential as self-assured, self-sufficient individuals and families. Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Counseling and therapy services include marriage, couples, and family. Focus areas include depression, anxiety, grief, chemical dependency, abuse, body image, spirituality, and life transitions. Offering services via telehealth.

Home Again sells used furniture and home furnishings, including mattresses. Does not sell televisions, large appliances, clothing of any kind or baby items. All proceeds go to Teen Challenge of the Dakotas. Free delivery/pick-up of all donations and purchases (within the city limits of Brookings).

Providing services and resources for Houseless Relatives including mail distribution to receive mail and packages once an individual completes a mail registration form, a Clothing Closet with essential clothing and personal items when available, daily access to Showers & Laundry (on a first-come, first-served basis), daily meal, food bags, and a Resource Center with computer and phone access for job applications, housing applications, and other resources. Housing Resources is an access point for Coordinated Entry.

Provides the food vouchers, prescription expense assistance (medicine only), vision expense assistance (eyeglasses and contacts), and personal care items (laundry soap, cleaning supplies, and hygiene items). Please note, prescription assistance is for those without insurance and it is for one time only! Personal care assistance is once in a calendar year.

Community outpatient mental health facility. Provides mental health services to children, adolescents and adults. Services include but are not limited to psychiatric care and evaluations, children's services, group and individual counseling, play therapy, aggression management, and more.

Provides various programs for catastrophically disabled veterans and other people with disabilities. Services include, but are not limited to the following areas: Service to veterans Veterans service officer - who is able to assist in obtaining benefits Advocacy programs for veterans and people with disabilities Legislation Peer counseling - for persons with physical disabilities and especially new spinal cord injuries and multiple sclerosis. Sports and recreational activities - Assists veterans that have physical disabilities with getting involved in sports activities such as bowling, lacrosse, boccia ball, fishing, skiing, trap shooting, and hunting. Oversees the backpack program for school age children and elderly food boxes for senior citizens (60 years and older). This institution is an equal opportunity provider.

Pharmacy services, vaccinations, and medical equipment/supplies are offered. All 3 locations serve as medication disposal sites.

Horizon Health - Elk Point is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT).
