Horizon Health - Huron is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, prenatal care, patient education, care management, DOT exams, immigration physicals, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Horizon Health - Huron also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Spanish and Karen translators available on-site.

Provides a variety of services including patient care, diagnostic services, support services, chemotherapy, emergency department, heart health, Planet Heart vascular and heart screening, nursing service, maternity care, medical/surgical, newborn nursery, pediatrics, respite care, swing bed, outpatient treatment, pain management, pregnancy and childbirth, rehabilitation services, cardiac rehab, occupational therapy, physical therapy, speech therapy, sleep study lab, surgical procedures, and telehealth.

Offering CPR/AED and First Aid courses, Lifeguard Course and Review, and Water Safety Instructor Training.

Services available include hospital (inpatient, outpatient and observation), swing bed (skilled nursing services), dietitian and nutrition services, rehabilitation services, laboratory services, X-ray and CT services, MRI, mobile mammogram and Dexa scan, clinic services, emergency services, and various outreach services. Also provides classes, programs, and events on a variety of toopics. These include tobacco cessation, cancer support group, CPR/First Aid Classes, fitness classes, and blood pressure monitoring.

Provides physical therapy, occupational therapy and speech therapy for individuals in need of rehabilitation. Please check with the location to see if provided. Must have a referral from a medical provider prior to evaluation and treatment depending on insurance requirements. The outpatient center is equipped to treat a wide variety of patients and diagnoses. There is also an occupational therapy driver's evaluation and training program available at the West 18th Street location.

K-5 (Kindergarten through 5th Grade) provides care and programming outside of classroom hours in several locations in Sioux Falls, Brandon and Harrisburg. Activities are designed to meet each age level's academic, social and emotional needs. Signature programs offer additional options for character and leadership, health and wellness, sports and recreation and Education/STEM/Arts focus. Scholarships are available for families in need.

Bright Start is a community-based, voluntary program which implements the evidence based Nurse Family Partnership model to provide nurse home visitation services to families during pregnancy, after delivery and may continue until the child's second birthday. Services include prenatal, maternal and infant/child health assessments and education; and health, safety and nutrition education. Families are linked with resources such as prenatal, postpartum, and pediatric medical care, family planning services, mental health services, job training, literacy services, employment opportunities, housing, substance abuse treatment, early childhood assessment and intervention opportunities, and other community resources

Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria. Provides help for older adults and people with disabilities.

Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.

Physical therapy treats injuries and diseases using therapeutic exercise and other hands-on techniques that focus on alleviating pain and improving posture, strength, endurance, balance, coordination and joint mobility. Occupational therapy treats issues that arise from a disruption in a person's occupations. Occupation is everything that people do to occupy themselves, including taking care of personal needs, activities that allow one to enjoy life, achieving developmental milestones or work. Speech-language therapy addresses issues with communication, memory, problem solving, swallowing, sound production, stuttering and voice for all ages.

Clinic services offered include Preventative Care, Pediatrics, Pregnancy Care, Women's Health, 3D Mammograms, Lab Work, and Imaging.

Programs and Services: First Time Homebuyer Education (Attendees may be eligible to receive discounts on closing costs.) Adult/Youth Financial Literacy and Debt Management (We teach tenants how to evaluate their current financial condition, budget their expenses, and set financial goals so they are able to pay their monthly rent on time.) Credit Review Services (We assist clients establish credit and improve credit scores.) Rental Education (Clients learn about tenant-landlord relationships, financial planning, credit restore & rescore.) Classes are offered by appointment either as an individual or in a group session (virtual, in person, or online course).

Providing health care and supportive services in a patient's home. Home Health may be considered for people who are recovering from an illness, injury, or surgery, undergoing treatment, people with disabilities, and those who are chronically ill or terminally ill. Services offered are skilled nursing care, home health aides for personal care, rehabilitation therapy (physical therapy, occupational therapy, and speech therapy), social services, patient/family education, medication monitoring, and nutrition consultation.

Mental health clinic addressing trauma in a safe and healthy way. Services offered include individual counseling, marriage counseling, divorce counseling, grief/bereavement counseling, Dialectical Behavioral Therapy (DBT), child abuse counseling, child abuse survivor counseling for adults, family counseling, couples counseling, trauma informed therapy, Cognitive Behavioral Therapy (CBT), adolescent/youth counseling, parent counseling, spouse/intimate partner abuse counseling, telemental health, traumatic incident reduction, and Eye Movement Desensitization and Reprocessing (EMDR).

Must be a resident of Lake County to qualify for assistance. Must fill out application in Welfare Office. Assistance is NOT immediate and must be approved by the County Commissioners at their next scheduled meeting after the application process. Poor relief assistance is the last resort. Must seek assistance from all other agencies and/or groups before applying. One-time assistance only in a 12-month period. Utilities: Must provide a notice of termination from the company/supplier. Available in limited amounts. Applicants must apply for Energy Assistance through the Office of Energy Assistance in Pierre, SD before we may assist. Maximum assistance of $420. Will not pay deposits, late fees or reconnect fees. Rent: Must provide an eviction notice. Housing assistance is available in limited amounts and is paid per scale of household members at a maximum of $500. Will not pay deposits, late fees, rent to immediate family members or rent for residents in subsidized housing. If resident must apply for SD Rental Assistance programs and/or low-income housing before the county may assist. Medical Bills: The hospital services received must constitute emergency, medically necessary services. The hospital and/or patient must demonstrate to the county that it has exhausted all avenues of payment, including the hospital accepting reasonable monthly payments from the patient. No assistance is available for one that has health care insurance. Non-emergency Services - the county does not cover this type of medical assistance. Such as non-emergency hospital/physician/specialist office visits and/or hospital services, medical equipment, medications, health insurance premiums, home health services, dental or eye care and childbirth. Any assistance approved creates a lien against the borrower/applicant and must be paid back to Lake County, SD.

ICAP Children and Family Services include the following: Half Day Services: Eligible children ages 3 - 5 will receive 3 1/2 hours of center-based preschool, 4 days per week, September - May. (limited locations) EHS Home Based Services: A home visitor meets with eligible women who are pregnant and/or families with children under the age of 3 once per week for 1 1/2 hours (at a regularly scheduled time). Group socialization meetings are held twice per month. Child Development Center: Madison and Sioux Falls (ages 4 weeks - 3 years) can receive early childhood services and childcare each day year-round at ICAP's childcare center.

White Buffalo Calf Woman Society provides emergency shelter and advocacy services to all victims of domestic violence, sexual assault, stalking, dating violence, and human trafficking.

Offers the following: books, audio books, movies, music CDs, library tours, meeting rooms, art displays, consumer information, auto repair information, tax forms, electronic databases, WIFI, free computer use, interlibrary loan service from other libraries, downloadable audio books and e-books, self-service checkout, and books delivered to the homebound and to senior care facilities. ​Programming and services for children including library materials and play spaces, story time programs, Lego Club, summer reading programs, special events, daycare delivery service, and outreach to local schools and child care centers. ​Weekly programming for Grades 7-12 including gaming, trivia, movies, and more. ​Adult programming that include book discussions, adult reading programs, educational and cultural speakers, events, and classes. ​Special collections include the Marge Tauber South Dakota Room (South Dakota and Watertown historical), genealogical books, large print books, Children's Educational kits, and Book Club Tubs.

Food Pantry services for those living in the Northern Turner County communities. There is a bread giveaway every 3rd Tuesday of the month held in the Turner County Courthouse basement.

Provides preschool scholarships to children from economically disadvantaged families in Rapid City. Families that are approved for a scholarship can choose from 7 different partnering preschools within the Rapid City area.
