The Extended Care and Rehab Service Line includes the following services: Caregiver support group, spinal cord injury / disorder program, full suite of therapies, chiropractic care, integrative health services, adaptive sports, low vision and visually impaired services, inpatient community living center, pain management, whole health services, community living center, hospice/palliative Care, polytrauma/traumatic brain injury (TBI), geriatrics & extended care (GEC), medical foster home, and home health care.
