Rosebud IHS provides the following mental health services: Individual, couples, marriage, family, and group counseling Psychological testing/mental health evaluation/bio-psycho-socials Medication management (Avel Telepsychiatry) Crisis intervention and Referral Mental health screening Grief and bereavement counseling Terminal illness counseling Utilize Best practice therapies, i.e., Cognitive Behavioral therapy counseling support for co-occurring disorders Teletherapy with Avel Telepsychiatry Also provides consultation to Rosebud Sioux Tribe Alcohol and Drug Abuse Treatment Inpatient and Outpatient Programs, the RST Methamphetamine Inpatient Treatment Program, Wanbli Wiconi Tipi Juvenile Detention Center, RST Adult Correctional Center, RST Courts System, Tiwahe Glu Kinipi Suicide Prevention Program, Wicozani Wakan Suicide Prevention Program, Tiwahe Glu Kinipi Systems of Care Youth and Family Mental Health Program, and local School Districts as requested. Suicide Prevention Specialist programming to assist with prevention activities.

Outpatient Services - Individual therapy (for mental health or substance use), Family therapy, Group therapy, Medication management, Psychological assessments and testing, Commitment evaluations, Nursing home consultations, and 24-hour a day crisis intervention via crisis line. Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Horizon Health - Faith is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Faith is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

Provides nursing facility and short-term rehabilitation care in a home-like environment with individualized care plans. Services include but are not limited to 24-hour nursing care, wound care management, nutrition services, safety systems, pain management, recreational activities, in-house pet therapy, physical/occupational/speech therapy, and restorative therapy. Also provides memory support, hospice, respite care services.

Provides quality home health care to residents of the Sioux Empire, Vermillion, and Chamberlain communities. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.

Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. Client groups include DBT (Dialectical Behavior Therapy) and MRT (Moral Reconation Therapy).

Provides pharmacy services such as prescription refills and transfers, medication therapy management, immunizations, durable medical equipment, long term care, and over the counter medications. This is a medical disposal take-back site. Customers can download the PocketRx mobile app to refill medications, set medication reminders, manage prescriptions, and have medications delivered to your door.

WIC, breastfeeding support, immunizations, school nursing for Colman and Flandreau, education in the community, TB medication management, car seats, promotion of safe sleep/pack-n-plays.

Pharmacy services include MEDpaks, Medicare Part D counseling, immunizations, and OTC medications. Customers can download the PocketRx mobile app to order medication refills, set medication reminders, and track status of medications.

Provides medical, mental health, and complementary services in one location to ensure the best treatment outcome. Services are available to all people of all ages seeking healing from mental, emotional, and physical injury to improve health and wellness. Services provided include individual counseling for children, teens, young adults, and adults. Specialized treatments include art therapy, sex therapy, EMDR, and addictions counseling. Treats many conditions such as anxiety, depression, trauma, grief, abuse, domestic violence, attachment issues, behavioral issues, suicidality, and more. Also provides counseling for couples and families, and formal addiction assessments and treatment and group therapy for recovery. Psychiatry services, including assessment, diagnosis, and medication management, are available for individuals ages 3 and older. Provides functional medicine, acupuncture, massage, and yoga services, as well as supervision for LPC-MH and CSW-PIP licensure and consultation for EMDR certification.

Provides prescription and over the counter medications, diabetes care, medication therapy management, PharmaSmart blood pressure monitoring program and vaccines. Vitamins and supplements are also available. Customers can download the Coborn's Pharmacy app to refill medications, set up reminders, and track your refill status.

Pierre location offers immunizations for Flu, Covid-19, RSV, and Shingles. Customers can download the PocketRx app to set medication/refill reminders, refill prescriptions, manage your family's medications, and communicate with pharmacist.

Offers mental health and substance use services as well as both family and individual case management services which include comprehensive child/adolescent/family services, comprehensive adult/family services, adult/adolescent/family substance abuse services, and outpatient psychiatric medical services. ​Walk-in assessments available during regular business hours and emergency mental health crisis support services available 24 hours a day / 7 days a week. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

Coteau des Prairies (CDP) Health Care System is a 25-bed, critical access acute care, community non-profit hospital serving approximately 21,000 residents in the Glacial Lakes Region in north east South Dakota, and west central Minnesota. Walk-in assessments are available at the Watertown and Sisseton locations. Also offers and telehealth services.
