Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. The Family Medicine Residency Clinic provides comprehensive care for the entire family in all phases of life, from childhood to senior years. Services include care for acute and chronic medical problems, care for expectant moms, well-baby visits and immunizations, school sports physicals, OB/GYN, prenatal care, annual check-ups, immunizations, and minor surgical procedures.

Provides a full range of primary healthcare services including physical exams, chronic disease management, acute illness management, physicals, well child exams, children/adolescent/men/women healthcare services, immunizations, telemedicine related to medical and mental health needs, certified laboratory services, All Women Count services, assistance with Marketplace insurance enrollment, and allergy injections.
