Horizon Health - Mission is a full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT).

Counseling for individuals, couples, groups, and families for all ages. Other services include play therapy, psychological assessments, psychotherapy, relationship counseling and limited neuropsychological assessments. This office is able to provide telehealth to anyone in the state of South Dakota. There is paperwork that must be completed and signed prior to the first appointment.

Responds to the needs of individuals in the Mitchell area affected directly or indirectly by mood-altering chemical use or dependency through a wide variety of services including treatment needs assessments, detoxification, intensive inpatient and outpatient treatment, first offender DUI courses, adolescent and adult aftercare, adolescent treatment, anger management, moral recognition therapy (MRT), relapse prevention courses, parenting classes, strengths group, CBISA, readiness group for federal probationers, referrals, group and individual therapy, transitional living, and continued care.

Provides a comprehensive range of state-accredited addiction counseling and mental health services. Services include: Treatment Needs Assessments (also known as drug and alcohol evaluations), individual counseling, couples counseling, and group counseling.

Outpatient mental health counseling center providing 100% free services to eligible Veterans, active duty service members, and their families. Part of the US Department of Veterans Affairs but separate from the VA Medical Centers, with separate and confidential medical records. Offering individual, group, and couples counseling both in Rapid City and at a number of Community Access Points in the Black Hills. Also, a referral service and can point Veterans in the right direction for any information they may need in terms of accessing benefits, enrolling in VA health care, etc. Specialty is helping Veterans readjust successfully into civilian life, and provides counseling for a wide range of problems, from mental health issues like PTSD, depression, and anxiety to life stressor issues such as work-related or relationship difficulties. Providing trauma-based counseling for adolescents 12 - 17 who have experience or are at risk for sexual exploitation or sex trafficking and parents, foster parents or caregivers of adolescent girls who have experienced or are at risk for sexual exploitation or sex trafficking using the following group coaching, counseling, advocacy and support services, safety planning, education and awareness, crisis response, skills classes, and recreational outings. All listed services are provided by pre-approval through referral form, according to available resources and as consistent with the mission statement.

Hope Harbor is committed to restoring families through a loving, bible-based, Christ-centered family program. Client base includes: Teens ages 12-17 Females in the Marshall, MN location; males in the Parker, SD location Reside within a reasonable weekly driving distance of the location Mild to moderate emotional and/or behavioral problems No alcohol or drug dependency No active suicidal threats No history of sexual offense or perpetration Not currently pregnant Must commit to program duration of 12-15 months.
