Horizon Health - Mission is a full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. The Counseling Center offers the following services (all services based on eligibility and priority needs): Substance abuse services include screening, intake, evaluation, referrals, aftercare, primary prevention programs, education, MRT/SBISA classes, and group counseling. ​Mental health services include couples therapy, family therapy, individual therapy, group therapy, Education, Referrals, and Psychological evaluations and services.

Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

Residential behavioral/group care program for abused and neglected youth offers youth 24-hour supervision. Services offered include individual therapy, treatment planning, mental health evaluation, group therapy, collaboration with parents/guardians/ foster parents, referral agency, schools or other provider, and educational services provided in program.

Provides workshops for schools and communities dealing with the topic of relational aggression, cyberbullying, human trafficking, social media safety and more.
