Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. Client groups include DBT (Dialectical Behavior Therapy) and MRT (Moral Reconation Therapy).

Also offering telehealth services.

CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders. Components of the program include residential services, independent living training, educational services, and therapeutic and psychiatric services (individual and group therapy).

Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Offers ART (Accelerated Resolution Therapy), social groups for children ages 10-12 and 18-21 who are neurodivergent. Experience in depression, anxiety, PTSD, life coaching, etc. Offers services via telehealth. Same day appointments are available for both counseling and psychiatry services.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Group therapies focus on developing coping skills through expressive therapies and behavioral therapy.
