Medical and Dental Clinics | City of Sioux Falls Health Department

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Counseling Services | Community Counseling Services

Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Provides medical, mental health, and complementary services in one location to ensure the best treatment outcome. Services are available to all people of all ages seeking healing from mental, emotional, and physical injury to improve health and wellness. Services provided include individual counseling for children, teens, young adults, and adults. Specialized treatments include art therapy, sex therapy, EMDR, and addictions counseling. Treats many conditions such as anxiety, depression, trauma, grief, abuse, domestic violence, attachment issues, behavioral issues, suicidality, and more. Also provides counseling for couples and families, and formal addiction assessments and group therapy for recovery. Psychiatry services, including assessment, diagnosis, and medication management, are available for individuals ages 3 and older. Provides functional medicine, nutrition therapy, massage, and yoga services, as well as supervision for LPC-MH and CSW-PIP licensure and consultation for EMDR certification.
