Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Provides medical, mental health, and complementary services in one location to ensure the best treatment outcome. Services are available to all people of all ages seeking healing from mental, emotional, and physical injury to improve health and wellness. Services provided include individual counseling for children, teens, young adults, and adults. Specialized treatments include art therapy, sex therapy, EMDR, and addictions counseling. Treats many conditions such as anxiety, depression, trauma, grief, abuse, domestic violence, attachment issues, behavioral issues, suicidality, and more. Also provides counseling for couples and families, and formal addiction assessments and group therapy for recovery. Psychiatry services, including assessment, diagnosis, and medication management, are available for individuals ages 3 and older. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. Client groups include DBT (Dialectical Behavior Therapy) and MRT (Moral Reconation Therapy).

A psychiatric hospital offering a wide range of services for geriatric, adult, adolescent, and child patients. This center approaches patient care with a special emphasis on trauma-informed care as well as biological, psychological, and social concerns. - Inpatient Treatment: Available for patients ages 5 and older. All patients will be psychiatrically assessed and treated by a licensed and board-certified psychiatrist. Additionally, experienced nurses and patient care technicians provide individual specialized attention 24 hours a day, 7 days a week. Inpatient Services: Specialized services include group and individual therapy, diagnostic testing, psychological evaluations, and family therapy. Recreational therapists provide a healing and therapeutic environment. Chemical Dependency Specialists perform detailed evaluations and treatment planning for patients struggling with addiction. Intensive case management is available to coordinate external services for the hospitalized patient. Hospitalization Process: If hospitalization is required, patients are provided a full physical examination and medical evaluation at the Rapid City Monument Hospital Emergency Department prior to hospitalization.
