Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Outpatient counseling (for Chemical Dependency and Co-Occurring Disorders) for adults 18 years and older including intensive outpatient treatment, continued care, Moral Reconation Therapy (MRT), and assessments.

Offers EMDR (Eye Movement Desensitization and Reprocessing). Telehealth sessions available. Also an LGBTQ+ friendly provider.

Services include alcohol and drug evaluations, referrals to inpatient or outpatient facilities, low intensity outpatient treatment, 6 week program, Moral Reconation Therapy, DUI Classes for Standing Rock Tribal Court only, Aftercare, adolescent program, community awareness programs, and Positive Indian Parenting.

An intensive, outpatient, 12-step recovery-based center for alcohol and drugs. Services are provided by certified chemical dependency counselors including assessments (walk-in assessments available), day and evening treatment programs, adolescent programs, 12 hour DUI class, Intensive Methamphetamine Treatment (IMT) offered with state approval only, and a Recovery Maintenance Group. This office is located close to the SAM (Sioux Area Metro) Orange bus route.

Outpatient services are available to anyone who may currently be experiencing problems which are of an emotional, behavioral, substance abuse, or interpersonal nature. Therapy services include individual therapy, group therapy, couples therapy, family therapy, home-based therapy, medication management, substance use individual and group treatment programs, crisis intervention, and telehealth. Diagnostic services include psychiatric evaluation, mental status examination, education assessment, emergency mental health evaluation, and treatment needs assessment for substance abuse problems. Offers walk in assessments for substance use disorder during regular business hours.

Behavioral Health Services available from licensed substance abuse and mental health counselors. ​Services include: chemical dependency assessments and Level 1.0 counseling, individual, couples, and family behavioral health counseling, Medication Assisted Therapy (MAT) for patients recovering from substance use disorders, EMDR (Eye Movement Desensitization Reprocessing) Therapy, and psychiatry team availability. Transportation is available for current patients if needed.

Offers Telehealth counseling services for rural South Dakotans to have access to an outpatient therapist via smart phone, tablet or computer. Call to schedule a session via confidential/secure HIPAA compliant video technology. Employee Assistance Program (EAP) - can provide trainings, classes/groups or counseling for employees. Youth prevention program focuses on issues of addiction, violence, promiscuity, and suicide. Other programs include: Lakota Circles of Hope, Disaster Relief and Uplifting Parents.

Provides equine-assisted counseling conducted in an arena with a team including a mental health professional, an equine professional, and one or more horses for individuals, couples, families, or groups. Intensive, non-residential treatment program providing services to children who meet the states' Severe Emotional Disturbance (SED) criteria and would not be successful in less restrictive services. Also provides services to their families. Incorporating counseling, case management, and family support services, with the goal of assisting in the development of a nurturing and stable family environment in which children may grow. A counselor along with family, individuals supporting the family, and professionals will develop a customized treatment plan which will assist in meeting the needs of the child and family. Services provided to program participants include 24-hour crisis assistance, counseling services, chemical dependency screenings and assessments, case management, dedication management, psychiatric services, classes and workshops on mental health topics, early childhood mental health services, and play therapy for children ages 0 to 5. Some therapies offered in this program include Dialectical Behavioral Therapy (DBT), Moral Reconation Therapy (MRT), and Aggression Replacement Therapy (ART).

Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.
