Provides drug and alcohol treatment for adolescents ages 13 - 17 years of age who are enrolled as a member of a federally recognized Tribe or able to show the lineal descendancy. It is a rolling admission and can be anywhere from 90 - 120 days.

Providers treatment for concerns including addiction, anxiety, depression, mood disorders, adjustment, abuse, spiritual development, emotional management, stress management, family issues, grief, trauma, personality disorders, and relational issues. Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

The Adolescent Addiction Recovery Unit provide inpatient and outpatient addiction recovery care for youth ages 12 to 17. Services include 24-Hour supervision, residential treatment, intensive counseling, and educational support.

An intensive, outpatient, 12-step recovery-based center for alcohol and drugs. Services are provided by certified chemical dependency counselors including assessments (walk-in assessments available), day and evening treatment programs, adolescent programs, 12 hour DUI class, Intensive Methamphetamine Treatment (IMT) offered with state approval only, and a Recovery Maintenance Group. This office is located close to the SAM (Sioux Area Metro) Orange bus route.
