Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

An intensive, outpatient, 12-step recovery-based center for alcohol and drugs. Services are provided by certified chemical dependency counselors including assessments (walk-in assessments available), day and evening treatment programs, adolescent programs, 12 hour DUI class, Intensive Methamphetamine Treatment (IMT) offered with state approval only, and a Recovery Maintenance Group. Provides a comprehensive range of state-accredited addiction counseling and mental health services. Services include: Treatment Needs Assessments (also known as drug and alcohol evaluations), individual counseling, couples counseling, and group counseling.

Outpatient services are available to anyone who may currently be experiencing problems which are of an emotional, behavioral, substance abuse, or interpersonal nature. Therapy services include individual therapy, group therapy, couples therapy, family therapy, home-based therapy, medication management, substance use individual and group treatment programs, crisis intervention, and telehealth. Diagnostic services include psychiatric evaluation, mental status examination, education assessment, emergency mental health evaluation, and treatment needs assessment for substance abuse problems. Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Offers primary care, mental health care, treatment for post traumatic stress disorder (PTSD), and suicide prevention. Also offers transitional and supportive housing for veterans experiencing homelessness. Gun safety locks are available through the Suicide Prevention Coordinator.

School-based alcohol and drug prevention and intervention counseling services for students and families. Services include: individual and group sessions, family counseling, drug and alcohol screening, tobacco screening, drug and alcohol assessments, referrals, aftercare (post treatment), and prescription drug lock boxes. Offering tele-mental health services. Must have access to computer with camera and microphone.

Intensive, non-residential treatment program providing services to children who meet the states' Severe Emotional Disturbance (SED) criteria and would not be successful in less restrictive services. Also provides services to their families. Incorporating counseling, case management, and family support services, with the goal of assisting in the development of a nurturing and stable family environment in which children may grow. A counselor along with family, individuals supporting the family, and professionals will develop a customized treatment plan which will assist in meeting the needs of the child and family. Services provided to program participants include 24-hour crisis assistance, counseling services, chemical dependency screenings and assessments, case management, dedication management, psychiatric services, classes and workshops on mental health topics, early childhood mental health services, and play therapy for children ages 0 to 5.
