Provides drug and alcohol treatment for adolescents ages 13 - 17 years of age who are enrolled as a member of a federally recognized Tribe or able to show the lineal descendancy. It is a rolling admission and can be anywhere from 90 - 120 days.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Behavioral Health Services available from licensed substance abuse and mental health counselors. ​Services include: chemical dependency assessments and Level 1.0 counseling, individual, couples, and family behavioral health counseling, Medication Assisted Therapy (MAT) for patients recovering from substance use disorders, EMDR (Eye Movement Desensitization Reprocessing) Therapy, and psychiatry team availability. Transportation is available for current patients if needed.

An intensive, outpatient, 12-step recovery-based center for alcohol and drugs. Services are provided by certified chemical dependency counselors including assessments (walk-in assessments available), day and evening treatment programs, adolescent programs, 12 hour DUI class, Intensive Methamphetamine Treatment (IMT) offered with state approval only, and a Recovery Maintenance Group. This office is located close to the SAM (Sioux Area Metro) Orange bus route.

This program serves male and female youth with a primary diagnosis of substance dependency disorder. Treatment philosophy recognizes substance dependency as a chronic and progressive illness. Treatment includes 12-step groups, Alcoholics Anonymous, and Narcotic Anonymous groups within a therapeutic milieu. Treatment is completed in approximately 45 days.

Outpatient Services - Individual therapy (for mental health or substance use), Family therapy, Group therapy, Medication management, Psychological assessments and testing, Commitment evaluations, Nursing home consultations, and 24-hour a day crisis intervention via crisis line. Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Individual, couple and family counseling for issues related to trauma, abuse, relationships, unplanned pregnancy, adoption and parenting. Offers Telehealth counseling services for rural South Dakotans to have access to an outpatient therapist via smart phone, tablet or computer. Call to schedule a session via confidential/secure HIPAA compliant video technology. Employee Assistance Program (EAP) - can provide trainings, classes/groups or counseling for employees. Youth prevention program focuses on issues of addiction, violence, promiscuity, and suicide. Other programs include: Lakota Circles of Hope, Disaster Relief and Uplifting Parents.
