Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Addiction services consist of: chemical dependency evaluations, intensive adult outpatient chemical dependency treatment, adult chemical dependent aftercare support groups, DUI classes, gambling treatment, prevention programs, Cognitive Behavioral Interventions for Substance Abuse (CBISA), tele-med services

Provides individual therapy and assessments for mental health and/or substance use. Serving adolescents and adults seeking support with anxiety, depression, mood disorders, adjustment disorders, life transitions, trauma, substance use disorders, etc. Telemental health available.

Individual, family and couple's counseling for mental health, psychiatry, chemical dependency, and ADHD assessment. Issues treated include anxiety, depression, mood disorders, adjustment disorders, substance abuse disorders, domestic abuse/violence, grief and loss, trauma, relational issues, and significant life changes. Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

Offers outpatient counseling services for individual, couple, and families. Staff members work with the client to provide services in the home, community, or office at the discretion of the client. Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Psychiatric services are also available to those over the age of 18 including assessments and medication management.

Individual and couples mental health and chemical dependency counseling to children, adolescents, and adults with a variety of issues including, but not limited to; depression, anxiety, obsessive compulsive disorder (OCD), physical and or sexual abuse, trauma, chemical dependency and other addictions, co-occurring disorders, family issues, behavioral issues, relationship issues, sexual identity, adjustment disorders, career issues, adoption issues, attachment issues, personality disorders, stress management, women's issues, eating disorders, post traumatic stress (PTSD), and crisis prevention. Offers EMDR (Eye Movement Desensitization and Reprocessing). Telehealth sessions available. Also an LGBTQ+ friendly provider.

An intensive, outpatient, 12-step recovery-based center for alcohol and drugs. Services are provided by certified chemical dependency counselors including assessments (walk-in assessments available), day and evening treatment programs, adolescent programs, 12 hour DUI class, Intensive Methamphetamine Treatment (IMT) offered with state approval only, and a Recovery Maintenance Group. This office is located close to the SAM (Sioux Area Metro) Orange bus route.

Provides drug and alcohol treatment for adolescents ages 13 - 17 years of age who are enrolled as a member of a federally recognized Tribe or able to show the lineal descendancy. It is a rolling admission and can be anywhere from 90 - 120 days.

South Dakota court approved treatment provider and provides court approved alcohol and drug assessments/evaluations and screenings including Individual 1st Offender DUI Education, Intensive Outpatient Services (IOP) and Aftercare. Offering Employee Assistance Program (EAP); Cognitive Behavior Therapy (CBT); Behavior Modification; Clinical Supervision; Reunification Therapy; Psychological Evaluations/Screenings; Parenting Coordination, Play Therapy, Moral Reconation Therapy, Substance Use Prevention, Assessments for Substance Use Disorders, Psychological Assessments, Fit for Duty Evaluations and First Responder Counseling. Individual, couple and family counseling for issues related to trauma, abuse, relationships, unplanned pregnancy, adoption and parenting. Offers Telehealth counseling services for rural South Dakotans to have access to an outpatient therapist via smart phone, tablet or computer. Call to schedule a session via confidential/secure HIPAA compliant video technology. Employee Assistance Program (EAP) - can provide trainings, classes/groups or counseling for employees. Youth prevention program focuses on issues of addiction, violence, promiscuity, and suicide. Other programs include: Lakota Circles of Hope, Disaster Relief and Uplifting Parents.

Individual and family therapy services specializing in eye movement desensitization and reprocessing (EMDR), trauma and abuse focused cognitive behavioral therapy (CBT), critical incident stress management, Thinking for a Change Group, and Love and Logic Parenting. Offers telehealth counseling through a secure video connection. Clients are not required to attend an intake session in person.

Outpatient services are available to anyone who may currently be experiencing problems which are of an emotional, behavioral, substance abuse, or interpersonal nature. Therapy services include individual therapy, group therapy, couples therapy, family therapy, home-based therapy, medication management, substance use individual and group treatment programs, crisis intervention, and telehealth. Diagnostic services include psychiatric evaluation, mental status examination, education assessment, emergency mental health evaluation, and treatment needs assessment for substance abuse problems. Offers walk in assessments for substance use disorder during regular business hours.

Provides assessments and therapy for individuals, groups, families, of all ages. Services address alcohol abuse and addiction, sexual abuse, spiritual abuse, marriage problems, family problems, adolescent and child adjustment, anxiety, grief, eating disorders, physical abuse, emotional abuse, delinquency problems, parent-child conflicts, stress, depression, spiritual needs, and mid-life issues. Offers telehealth sessions only if it is covered by insurance and client is located in state at the time of the call. Preference is for the first appointment to be face-to-face.

This program serves male and female youth with a primary diagnosis of substance dependency disorder. Treatment philosophy recognizes substance dependency as a chronic and progressive illness that can interfere with a person's growth from childhood to young adulthood. Treatment includes 12-step groups, Alcoholics Anonymous, and Narcotic Anonymous groups within a therapeutic setting. Treatment is completed in approximately 45 to 60 days.

Walk in assessments available during regular business hours at the Yankton and Vermillion locations. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Intensive, non-residential treatment program providing services to children who meet the states' Severe Emotional Disturbance (SED) criteria and would not be successful in less restrictive services. Also provides services to their families. Incorporating counseling, case management, and family support services, with the goal of assisting in the development of a nurturing and stable family environment in which children may grow. A counselor along with family, individuals supporting the family, and professionals will develop a customized treatment plan which will assist in meeting the needs of the child and family. Services provided to program participants include 24-hour crisis assistance, counseling services, chemical dependency screenings and assessments, case management, dedication management, psychiatric services, classes and workshops on mental health topics, early childhood mental health services, and play therapy for children ages 0 to 5. Some therapies offered in this program include Dialectical Behavioral Therapy (DBT), Moral Reconation Therapy (MRT), and Aggression Replacement Therapy (ART).
