Neurologists and physiatrists at Monument Health Neurology and Rehabilitation treat disorders of the central nervous system and neuromuscular system, which include the brain, spinal cord, nerves, and muscles. Our providers specialize in Alzheimer's disease and other neurodegenerative diseases, headaches, multiple sclerosis, pain management, physical medicine and rehabilitation, seizure disorders, sleep medicine, and strokes.

Primarily provides food to those in need. Also provides the following when available - gas cards, diapers, personal hygiene items, and cleaning supplies.

Medical assistance programs include: ​Children's Health Insurance Program (CHIP). Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. A full-service medical clinic with lab x-ray services onsite as well as wellness exams, vaccinations, and procedures.

Serves as a safe, productive, and engaging place for youth to go after school and all day during the summer on week days. Offers a drop-in facility and enrichment programs for youth Junior Kindergarten - 12th grade. Youth receive a free daily snack and a hot evening meal and participate in programs focused on academic success, healthy lifestyles, character, and leadership. During the summer months, free breakfast and lunch is served to any child 18 and younger.​

Bright Start is a community-based, voluntary program which implements the evidence based Nurse Family Partnership model to provide nurse home visitation services to families during pregnancy, after delivery and may continue until the child's second birthday. Services include prenatal, maternal and infant/child health assessments and education; and health, safety and nutrition education. Families are linked with resources such as prenatal, postpartum, and pediatric medical care, family planning services, mental health services, job training, literacy services, employment opportunities, housing, substance abuse treatment, early childhood assessment and intervention opportunities, and other community resources

Provides mental health therapy via telehealth services for individuals, couples, and adolescents. Areas of specialty include anxiety, depression, trauma, relationship challenges, and LGBTQ+ empowerment. Sessions are tailored to meet clients personal needs in a secure, confidential, and convenient virtual environment.

Food giveaway, once a month. Distributes food for which income guidelines apply (this institution is an equal opportunity provider). Community meal also served on the day of the food giveaway.

A joint effort of the Scottish Rite Foundation of South Dakota and the Communication Sciences and Disorders Department at USD with support from the Sioux Empire United Way. The clinic's purpose is to provide evaluation, treatment, and outreach services for children birth to 21 years in the Sioux Falls area with speech, language, and literacy deficits. Also provides a training site for graduate students in Speech Language/Pathology. Examples of disorders seen include children who are late talking, who have articulation disorders, language disorders, stuttering difficulties, hearing losses, etc. The clinic also serves as a diagnostic center for written language / dyslexia. There is a minimal fee for services; no one is denied due to an inability to pay. Referrals are taken from parents, professionals, and family members.

Food pantry for Hot Springs area provides a pre-packed bag of food, which consists of canned vegetables, fruit, beans, tuna, chicken, soup as well as milk and pasta. Clients also receive bread and meat items. In addition, clients may select from other items as available on the pantry shelves. This ministry is supported (monetarily and with food donations) by the community of Hot Springs.
