Horizon Health - De Smet Medical is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT).

Provides primary LGBTQ+ health care for people up to the age of 21. This includes routine check-ups, sexually transmitted/HIV/pregnancy tests, and sexual health education. An in-house pharmacy is available as well as mental health counseling services. Dr. Amanda Diehl has more than 20 years of experience and trained on LGBTQ+ health care through the Fenway Institute's National LGBT Health Education Center. In addition, the entire staff has been trained to provide an affirming atmosphere for the LGBTQ+ population.

Responsible for commissioners proceedings, agendas, and records, accounting records, revenue, and expense reports, voter registration and election, businesses applications for beer and liquor licenses, figures real estate and mobile home taxes, handles county bid lettings, manages medical indigent, county payroll and employee drug and alcohol testing programs, and the Auditor and Treasurer work in conjunction to pay county bills. This office also oversees one time, last resort/emergency financial assistance in the form of an interest-free loan for residents who need help with burial expenses, medical indigent claims, prescription payment assistance, rent payment assistance (to prevent eviction), and utility payment assistance for current and past due bills regarding water, lights, heating, phone, and trash.

Provides short or long term rental assistance to persons who are diagnosed with HIV/AIDS and meet income limitations. Must be behind (i.e.: if rent is due on the 5th, come for assistance after the 5th). Eviction notice is not required, however, there will be a form the landlord needs to complete the verify rent is past due.

Services include: mental health/addiction evaluations, screenings and treatment services for addiction, adjustment disorders, anger management, anxiety, attention deficit hyperactivity disorder (ADD/ADHD), Asperger's Syndrome, behavior problems, conduct concerns, depression, grief, mood disorders, oppositional defiant disorder, obsessive compulsive disorder (OCD), parent-child conflicts, post-traumatic stress, school refusal, separation and divorce, family issues, social skills training and stress management. South Dakota court approved treatment provider and provides court approved alcohol and drug assessments/evaluations and screenings including Individual 1st Offender DUI Education, Intensive Outpatient Services (IOP) and Aftercare. Offering Employee Assistance Program (EAP); Cognitive Behavior Therapy (CBT); Behavior Modification; Clinical Supervision; Reunification Therapy; Psychological Evaluations/Screenings; Parenting Coordination, Play Therapy, Moral Reconation Therapy, Substance Use Prevention, Assessments for Substance Use Disorders, Psychological Assessments, Fit for Duty Evaluations and First Responder Counseling. Virtual and in-office sessions available.

Committed to offering safe and confidential services. Provides individual, couples, and family services for those experiencing personal, relational and psychological issues. Telehealth services available.

Individual therapy for ages 14 and up. Topics discussed include: anxiety, depression, trauma, PTSD, chronic pain, stress management, employment issues, abuse, grief and loss, LGBTQ+ counseling, and women's issues. Telehealth sessions available.

Clinic locations offer family care to Sioux Falls and the surrounding area. Provides a variety of services for family and primary care (including flu shots) as well as prenatal care and OB/GYN. Other health services include yearly checkups, treatment of minor illnesses, and management of chronic conditions.

A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

Provides both economic and medical assistance to residents of Beadle County on a one time basis. Economic assistance is for current amounts only (no overdue bills). No payment for deposits of any kind. Rent, Utility assistance for current bills may be available for water, lights, and heating, and Medical, hospitalization, and burial assistance requires pre-approval if not for emergency services. All other resources must have been exhausted first. Assistance does require payback and county lien is filed until paid in full.

Provides food and/or personal hygiene items along with cleaning supplies to those that are in need of assistance.

Provides the food vouchers, prescription expense assistance (medicine only), vision expense assistance (eyeglasses and contacts), and personal care items (laundry soap, cleaning supplies, and hygiene items). Please note, prescription assistance is for those without insurance and it is for one time only! Personal care assistance is once in a calendar year.

Addiction services consist of: chemical dependency evaluations, intensive adult outpatient chemical dependency treatment, adult chemical dependent aftercare support groups, DUI classes, gambling treatment, prevention programs, Cognitive Behavioral Interventions for Substance Abuse (CBISA), tele-med services

Founded on the belief that everyone deserves a home. Pathways offers four different shelter programs:

• Emergency Shelter for Families:

8 rooms designated for households with children. Families will receive 30 days for free in a 12-month period. After 30 days, rent will be charged for extended stay.

• Tier 2 Services:

2 rooms designated for men and women seeking a more permanent residence and access to more services. To qualify for this program, individuals must be employed or be able to pay rent at the start. Total of 4 beds for women and 4 beds for men.

• Re-Entry Program:

Partnership with South Dakota Department of Corrections; provides 8 beds for men who are needing to re-enter the community. These individuals must follow the same guidelines for admission as other clients. Young Adult Transition Program: Provides transitional living and case management for young adults 18 to 24 years old who are experiencing homelessness. 4 beds are available for this purpose and can help people within the age range struggling in the community. Contact Staff for more details. Through the Emergency Solutions Grant, Pathways offers two programs:

• Homeless Prevention

For individuals and families who are unable to pay their rent or utilities and will otherwise be displaced from their residence. Call staff to learn more about eligibility requirements.

• Rapid Re-housing

For individuals who have found potential housing but may not yet have the funds to secure that housing. For those who are eligible, Pathways may be able to pay first month's rent, rental deposit and utility deposit. Call staff to learn more about eligibility requirements. Funding through the Security Deposit Assistance Program allows Pathways to operate a program solely to assist with security deposits.

Primary care clinic providing family and internal medicine to those without insurance. Specifically, treating chronic and acute conditions including diabetes, high blood pressure, cholesterol, indigestion, physicals, colds/cough/flu, eat and throat infections, sexually transmitted disease screening and treatment, and immunizations.

Assists children and youth in Brookings County who may not have food on the weekends by providing easy-to-prepare meals and snacks throughout the school year and summer months. Food is distributed through the schools on Fridays and is available for pick up in the summer. This organization is an equal opportunity provider.

Provides a variety of services to assist adults with disabilities. Programs include residential services, community support services, a variety of day services, employment services, family support program, and shared living.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Pharmacy Services include private consultation, medication therapy management services, home and mail delivery, durable medical equipment, medication synchronization, compounding, over the counter medications, and first aid supplies. Pierre location offers immunizations for Flu, Covid-19, RSV, and Shingles. Customers can download the PocketRx app to set medication/refill reminders, refill prescriptions, manage your family's medications, and communicate with pharmacist.

Provides physical therapy, occupational therapy and speech therapy for individuals in need of rehabilitation. Please check with the location to see if provided. Must have a referral from a medical provider prior to evaluation and treatment depending on insurance requirements. The outpatient center is equipped to treat a wide variety of patients and diagnoses. There is also an occupational therapy driver's evaluation and training program available at the West 18th Street location.

Provides mutual coordination, cooperation, and communication among foster parents, agency representatives, and the community to improve the foster care system and enhance the lives of foster children and foster families. Raises awareness of current issues, increasing community awareness of the foster care program, and the needs for foster families, providing training and continuing education for foster parents in order to improve recruitment, retention, and the foster family effectiveness. Provides information and referral services to organizations that host support group meetings for foster families and connects them to community organizations that can support families.
