Founded on the belief that everyone deserves a home. Pathways offers four different shelter programs:

• Emergency Shelter for Families:

8 rooms designated for households with children. Families will receive 30 days for free in a 12-month period. After 30 days, rent will be charged for extended stay.

• Tier 2 Services:

2 rooms designated for men and women seeking a more permanent residence and access to more services. To qualify for this program, individuals must be employed or be able to pay rent at the start. Total of 4 beds for women and 4 beds for men.

• Re-Entry Program:

Partnership with South Dakota Department of Corrections; provides 8 beds for men who are needing to re-enter the community. These individuals must follow the same guidelines for admission as other clients. Young Adult Transition Program: Provides transitional living and case management for young adults 18 to 24 years old who are experiencing homelessness. 4 beds are available for this purpose and can help people within the age range struggling in the community. Contact Staff for more details. Through the Emergency Solutions Grant, Pathways offers two programs:

• Homeless Prevention

For individuals and families who are unable to pay their rent or utilities and will otherwise be displaced from their residence. Call staff to learn more about eligibility requirements.

• Rapid Re-housing

For individuals who have found potential housing but may not yet have the funds to secure that housing. For those who are eligible, Pathways may be able to pay first month's rent, rental deposit and utility deposit. Call staff to learn more about eligibility requirements. Funding through the Security Deposit Assistance Program allows Pathways to operate a program solely to assist with security deposits.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

Gives out food and household necessities, including frozen meat, toilet paper, and paper towels.

Medical assistance programs include: ​Children's Health Insurance Program (CHIP): Coverage includes such things as doctor, hospital, prescription drugs, counseling, dental and glasses. Insurance for children up to age 19 at no cost to the family if the income is below certain income levels found on the Federal Poverty Guidelines. If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria. Provides help for older adults and people with disabilities.

Provides outpatient counseling services for individuals ages 18 - adulthood. Specialties include anxiety, adult ADHD, adult neurodivergent, stress, grief, compassion fatigue and burnout among professionals. Telehealth services are available for new and existing clients who live in South Dakota. Call or email for information.

Provides the following services - Advocacy: Teaches people with disabilities the skills needed to advocate for themselves. Independent Living Skills: Provides hands-on training in daily-living skills including cooking, budgeting, organizing, and more. Information and referral: Connects people with resources which can provide needed information to assist individuals to live independently. Peer counseling: Provides opportunities for people with disabilities to engage with others in small or large groups or on an individual basis. Transition: Facilitates the transition of individuals with disabilities from a nursing home to independent living; providing prevention services to individuals who choose to live independently and not in a nursing home or assisted-living setting; and assisting youth to gain skills as they transition into adulthood. Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

A monthly ecumenical worship service with children's activities. Each service is hosted by a ministry partner. Worship is followed by a community meal/picnic. Hygiene kits, and snack bags are available based on supply. Need not attend worship to receive the meal, hygiene kits, and snack bags. All are welcome. Need a ride? Clinic locations offer family care to Sioux Falls and the surrounding area. Provides a variety of services for family and primary care (including flu shots) as well as prenatal care and OB/GYN. Other health services include yearly checkups, treatment of minor illnesses, and management of chronic conditions.

Provides occupational and speech therapy services to children of all ages who have developmental delays, sensory processing challenges, speech and language delays, or difficulties with emotional regulation. Therapists provide mobile services to support children in their natural environments (home or daycare settings) as well as at the clinic location. The team also partners with early childhood educators to provide staff training and developmental screenings to support early identification and intervention.

Recycles donated clothing, furniture, household goods, books, and other items by offering them for purchase. Proceeds benefit the community by offering individuals with barriers work training and employment opportunities.

Provides clinic services such as primary care, women's health, men's health, physicals, preventative medicine, pediatric care, enhanced laboratory services, immunizations, and health education.

Featured health services include AveraNow, kidney care, behavioral health, children's health, dermatology, laboratory, pharmacy, medication disposal, primary care, surgery, urgent care, and urology.

Provides services for individual, couples, children, and family counseling as well as anger evaluations, and anger management. Committed to helping people who are experiencing emotional, psychological, personal, or family problems to increase hope and fulfill their treatment goals for a better life.

Provides insight and information regarding typical behaviors and the stages of development in children. From birth to their first day of school and beyond, Avera's Family Life Educators are available to help every step of the way. Offers a variety of training opportunities for parents, babysitters, and child care centers/providers. Training and classes include personalized Childcare Provider Training, Babysitting Classes for Tweens aged 10-13, and Home Alone Class for children ages 9-13. The Temperament Project can help better understand a child and can help choose appropriate techniques that fit a child's unique approach to life. Completing a Temperament Profile will give an individualized graph containing details of the child's characteristics in each of the 9 Temperament Traits. ICAP Children and Family Services include the following: Half Day Services: Eligible children ages 3 - 5 will receive 3 1/2 hours of center-based preschool, 4 days per week, September - May. (limited locations) EHS Home Based Services: A home visitor meets with eligible women who are pregnant and/or families with children under the age of 3 once per week for 1 1/2 hours (at a regularly scheduled time). Group socialization meetings are held twice per month. Child Development Center: Madison and Sioux Falls (ages 4 weeks - 3 years) can receive early childhood services and childcare each day year-round at ICAP's childcare center.

A year round, non-profit dinner theatre with gluten free and vegetarian entree options available. Summer kids camp available.

Provides the food vouchers, prescription expense assistance (medicine only), vision expense assistance (eyeglasses and contacts), and personal care items (laundry soap, cleaning supplies, and hygiene items). Please note, prescription assistance is for those without insurance and it is for one time only! Personal care assistance is once in a calendar year.

A private, non-profit, community-based human services agency service. Therapists, counselors, and case managers provide: Individual and family counseling (e.g., marriage counseling, parenting skills, trauma informed services, EMDR, depression, anxiety, PTSD, anger management, coping skill development, etc) Addiction and dependency services Evaluations and assessments Social learning programs Systems of Care Prevention services, providing presentations to student and adult groups on a large number of topics and consists of a library that contains over 1,000 items available for check out at the main Lemmon office Offering services via telehealth. Intensive, non-residential treatment program providing services to children who meet the states' Severe Emotional Disturbance (SED) criteria and would not be successful in less restrictive services. Also provides services to their families. Incorporating counseling, case management, and family support services, with the goal of assisting in the development of a nurturing and stable family environment in which children may grow. A counselor along with family, individuals supporting the family, and professionals will develop a customized treatment plan which will assist in meeting the needs of the child and family. Services provided to program participants include 24-hour crisis assistance, counseling services, chemical dependency screenings and assessments, case management, dedication management, psychiatric services, classes and workshops on mental health topics, early childhood mental health services, and play therapy for children ages 0 to 5. Some therapies offered in this program include Dialectical Behavioral Therapy (DBT), Moral Reconation Therapy (MRT), and Aggression Replacement Therapy (ART).

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.
