Provides long-term care and short-term rehab. Long term care services include 24-our nursing services, palliative care, memory assist, complex care, dining services, continuum of care services, and rehabilitation services. Short-term rehab services include physical therapy, cardiac/neurological/orthopedic recovery and rehabilitation, injury and illness rehabilitation, post hospitalization care, pre-scheduled post surgery care, pulmonary rehabilitation, and complex would care with an outcome-focused approach.

A public health nutrition program for income-eligible infants and children up to age 5, pregnant, postpartum, and breastfeeding women. WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

Responsible for commissioners proceedings, agendas, and records, accounting records, revenue, and expense reports, voter registration and election, businesses applications for beer and liquor licenses, figures real estate and mobile home taxes, handles county bid lettings, manages medical indigent, county payroll and employee drug and alcohol testing programs, and the Auditor and Treasurer work in conjunction to pay county bills. This office also oversees one time, last resort/emergency financial assistance in the form of an interest-free loan for residents who need help with burial expenses, medical indigent claims, prescription payment assistance, rent payment assistance (to prevent eviction), and utility payment assistance for current and past due bills regarding water, lights, heating, phone, and trash.

Investigates child abuse and neglect cases by working with the State's Attorney, law enforcement, and community resources to determine extent of alleged child abuse or neglect. Other services include adoptions, licensing of foster homes, assessment services, case management, and intensive family services. Offers the following services to the community: Clothing Closet (can be utilized once every 6 months except in special circumstances) and a Thrift Store (clothing available at all times for very reasonable prices). The store does sell mattresses and other furniture, when available.

6-week class for parents with children ages birth - 3 years old. Parents will learn about topics such as social-emotional growth, early brain development, temperament, and guidance.

Serving the citizens of the greater Canton area who are not close to a food pantry. This service is available to those in need up to 1 time per month. This pantry does not have any baby items available (i.e. diapers, formula, or baby food).

St. John's Community Clothing Room offers clothing for all ages, coats, shoes, hats and other wearables, bedding and towels. All items are free.

Those in need can find new and gently used items for a minimal cost. The store offers everything from furniture, household, and crafts items to clothing for men, women and children. Earnings from the store are used to directly assist those in need by helping pay rent, utilities, food, clothing, household, transportation for work and medical, prescriptions, and other necessities.

Services include: individual counseling, youth counseling, parent counseling, LGBTQ+ counseling, gender identity counseling, grief/bereavement counseling, dialectical behavioral therapy (DBT), eye movement and desensitization reprocessing therapy (EMDR), brain spotting and internal family systems, and telemental health counseling. Areas of focus include teenagers, adults, anxiety, depression, suicidal ideation, impulsivity, self harm, personality disorders, life transitions, relationship concerns, coping skills, and stress.

Provides food to those who are food insecure in the Yankton School District through 2 programs: The Sack Pack Program provides packs on a weekly basis to school aged children in need of food over the weekend. Families register for Sack Pack at the beginning of the school year. The school staff help distribute the packs. The School Food Pantry is open to any families that are food insecure with children attending school in the Yankton school district. The pantry is open once a month usually on the 1st Tuesday of the month.

A youth organization that provides a safe place for kids to go after school and during the summer on week days. Members participate in a variety of activities such as homework help/tutoring, organized sports programs, Youth for Unity, Teen Center, and guided activities on character and leadership, education and career skills, and healthy living/active lifestyles. Snacks are provided.

Allows parents to legally surrender newborns to designated safe places. State law allows a parent to voluntarily leave their baby if the child is less than 60 days old and the child has not been harmed. Key provisions of this program include: the parent may be asked for medical information related to the baby's medical history; identification of themselves or the other parent are not required; leaving the baby at a Safe Haven location is not a crime; if a parent doesn't express intent to return for the baby, their parental rights will be terminated after 14 days. Those providers and agencies a parent may leave their baby with under this law are hospitals and clinics, law enforcement officers, licensed child placement agencies, any Department of Social Services office, emergency medical technicians, and firefighters.

Mobile Vet Center (MVC) are large mobile vehicles with space for confidential counseling and are used to provide outreach to individuals in communities that are distant from existing services. Also provides emergency services in response to national emergencies and disasters.

Provides pharmacy services such as compounding, immunizations, delivery service, multi-dose compliance packaging, durable medical equipment, and an medication disposal site. Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications.

Retail pharmacy and a medication disposal site. Services provided are immunizations, medical equipment/supplies, smoking cessation, and pain management. Also sells OTC medications.

Inpatient treatment with units dedicated for children, adolescents, adults and seniors with acute psychiatry needs. A treatment team, headed by a psychiatrist, works to evaluate current symptoms and develop a positive treatment plan to address concerns. Treatment services include, but are not limited to depression, anxiety, addictive disorders, bipolar disorder, personality disorders, and schizophrenia. Group therapies focus on developing coping skills through expressive therapies and behavioral therapy.

The Center provides a nutritious, freshly made meal served 11:00am - 12:30pm, Monday - Friday at an affordable price. Over 48 activities that are lead by volunteers. Programs include table games (cards, cribbage, mah jongg, Mexican train dominoes), creative writing, pickleball (6 outdoor courts, 3 indoor courts), pool room, fitness area and more.

A non-profit, pay-what-you-can cafe. People may donate for the meals by cash, credit or volunteer an hour for their meal. There is a $3.00 minimum to select an item from the menu. If individuals wish to volunteer for their meal, volunteering is at 8:00am or 1:00pm. Information for non-profits wanting to make a referral: Family meal referrals can be requested by local non-profit organizations 48 hours prior, details needed include: family name, contact number, number in household & distribution information. These meals include 3 meals for the entire family. Pick up will be available from 11:00am - 1:00pm, Monday - Friday Fork Real works with individuals needing to complete Community Service hours.
