A non-profit, pay-what-you-can cafe. People may donate for the meals by cash, credit or volunteer an hour for their meal. There is a $3.00 minimum to select an item from the menu. If individuals wish to volunteer for their meal, volunteering is at 8:00am or 1:00pm. Information for non-profits wanting to make a referral: Family meal referrals can be requested by local non-profit organizations 48 hours prior, details needed include: family name, contact number, number in household & distribution information. These meals include 3 meals for the entire family. Pick up will be available from 11:00am - 1:00pm, Monday - Friday Fork Real works with individuals needing to complete Community Service hours. Call to arrange a time. 'Day Of' Service hours needed cannot be accommodated.

Monthly gatherings consists of trauma-informed education, small group sharing, dinner, and childcare to remind adoptive and foster families that they aren't alone and healing is possible.

Provided to prevent shut-off of current residence utilities only (not for deposits or previous addresses). Assistance is for a one-time emergency situation, not on a yearly basis. Client must have tried seeking assistance from Minnehaha or Lincoln County Human Services prior to caseworker visit. The maximum assistance provided is capped at $250.00. Horizon Health - Faith is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Faith is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

Provides individual, couples/marriage, and family counseling. Topics covered include depression, anxiety, obsessive compulsive disorder (OCD), attention deficit/hyperactivity disorder (ADHD), adjustment disorders, bipolar, post traumatic stress disorder (PTSD), child sexual abuse, domestic violence, marriage issues, spiritual issues, women's issues, church leadership abuse issues, Asperger's, social skills, self-esteem, bullying, and individuals whose spouses have addiction issues.

Provides short or long term rental assistance and mortgage payment assistance to persons who are diagnosed with HIV/AIDS and meet income limitations. Providing free breakfast for children 18 years old or younger. Meals on Wheels Western South Dakota will fund breakfast at no charge for seniors (60+) accompanying their grandchildren. ICAP Emergency Services helps with emergency utilities and rent (Not available to those residing in Sioux Falls) for income qualified households. Must meet all requirements. Also offers food pantry vouchers to those who qualify.

Provides assisted living care, independent living, memory care, skilled care, and rehabilitation services in a community setting. Services include personalized care planning, nutrition services, activities for socialization, 24-hour staffing, and medication assistance.

Provides services for individual, couples, children, and family counseling as well as anger evaluations, and anger management. Committed to helping people who are experiencing emotional, psychological, personal, or family problems to increase hope and fulfill their treatment goals for a better life.

Offering acute care to patients with the following symptoms: rashes/minor burns or scrapes/bumps; UTI (urinary tract infections); cough; cold; allergies; asthma; fever; ear and sinus pain; nausea, vomiting, or diarrhea; sore throat; pink eye; and COVID-19 screening. Also offers primary care, chronic disease management, pre-surgical visits, medical weight management, physical therapy, and occupational therapy. Offers appointments via telehealth.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

The Health Cooperative is here to partner with patients to stay healthy and meet their health goals. An inter-professional team is available to offer early evaluations of adult health concerns. Also provides personalized self-care education that will assist patients in making healthier choices. This is not a replacement for a primary care doctor. Works alongside patients and their doctors to improve patient health. Team consists of registered nurses, a pharmacist, an occupational therapist, a physical therapist and a dietitian.

Providing clinical health services for all ages, including Medications for Opioid Use Disorder, senior care, maternity, pediatric, men's health, LGBTQ+ care, vaccinations, lab services, and telehealth visits. A Community Health Worker is also available to help coordinate services.

Psychiatric Services provide evaluations and medication management for youth and adolescents, working with therapy and case-management teams.

K-5 (Kindergarten through 5th Grade) provides care and programming outside of classroom hours in several locations in Sioux Falls, Brandon and Harrisburg. Activities are designed to meet each age level's academic, social and emotional needs. Signature programs offer additional options for character and leadership, health and wellness, sports and recreation and Education/STEM/Arts focus. Scholarships are available for families in need.

A free, relationship enrichment program open to the public. It serves individuals, couples, and families. The goal of the program is for participating adults to develop the skills they need to experience healthy relationships and improve their path toward financial wellness and employability. ​Relationship education and family support topics include: communication skill, controlling emotions, conflict resolution, making wise choices, setting life goals, mentorship, stress management, advocacy, and parenting strategies. ​Financial wellness/employability services include: financial literacy, job search assistance, money management, joint budgeting, interview practice, resume coaching, and job skills and interest assessments.

The county is a resource of last resort, available only after all other resources have been exhausted. Any applicant who receives assistance will have a lien filed in their name for that amount and will be responsible for repayment of that lien to the County. Requests for assistance with:

Partial assistance with the current month's rent. Rental deposits are limited to the availability of grant funds.

• Mortgage

Partial assistance of one month's payment if applicant meets program qualifications.

• Utilities

Partial assistance with past due bills for water, electric, natural gas and propane usage. Additional utility charges and fees limited to the availability of grant funds.

• Transportation Costs

Assistance with bus passes or gasoline for verifiable medical appointments or new employment.

• Medical

Assistance may include emergency prescription medications, specialty physician services, hospitalizations, and insurance premiums. Services may require prior approval.

• Burial or Cremation

Pennington County residents without resources or family members to assist.

Offers family counseling and adolescent/youth counseling.
