K-5 (Kindergarten through 5th Grade) provides care and programming outside of classroom hours in several locations in Sioux Falls, Brandon and Harrisburg. Activities are designed to meet each age level's academic, social and emotional needs. Signature programs offer additional options for character and leadership, health and wellness, sports and recreation and Education/STEM/Arts focus. Scholarships are available for families in need.

Community Living Services: Assists individuals in obtaining accessible, affordable housing, transportation, and other community living services. Home Modifications Adaptive Devices: Provides home modifications and adaptive devices, which could include ramps, lift chairs, bathroom modifications, reachers, canes, walkers and more. Telecommunication Adaptive Devices: Provides adapted telecommunication services, which includes an emergency phone dialer. American's with Disabilities Act: Serves as educators and consultants for contractors, businesses, organizations, and individuals regarding the American with Disabilities Act (ADA) and its guidelines and requirements. Assistive Daily Living Services: Provides services for people ages 18 and older, with physical disabilities affecting all four limbs, who need assistance with daily living activities. Services include program education and support and personal attendant services. Home Again South Dakota: Provides options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

• 1 bag that contains 20 diapers and a package of wipes.

​Only diapers sizes 1-5 are available. Each person receives one bag per visit.​ The program is limited, as it is donation based and services are not guaranteed. Individuals can visit once a week.

Children's Connection works with the Sioux Falls School District to provide group counseling services to children with an incarcerated loved one. Family Connection, through the Children's Connection program, is working to break the generational cycle of incarceration. The program involves group counseling and other appropriate services with an emphasis on providing support, education and other positive coping skills. The goal is to provide a safe place for children to talk and know they are not alone.

Provides a full range of treatment and rehabilitation services for adults with severe mental illness. The goal is to prevent hospitalization and to provide support and education; which, in turn, promote optimal levels of independence in living and working in the community. Services include psychiatric and medication management services, CARE (continuous assistance with recovery and empowerment), case management, supportive counseling, assisted living, vocational services, homeless outreach program, and IMPACT (individualized mobile program for assertive community treatment) services. Client groups include DBT (Dialectical Behavior Therapy) and MRT (Moral Reconation Therapy).

A skilled nursing community offering transitional short-term rehabilitation, long-term nursing facility care, hospice care, and respite care. Provides individualized care for residents and patients to return to higher levels of functioning in a home-like community-based setting aimed maximizing residents capabilities and quality of life.

WIC provides nutrition education to help make healthy eating choices, breastfeeding education for expectant and new mothers looking to learn more about their infant nutrition options, referrals to connect families with quality medical care and other community services, and food benefits to help families purchase nutritious groceries at participating businesses statewide.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

The Huron Psychiatric Residential Treatment Facility (PRTF) serves male adolescents ages 10-17. The program focuses on helping them develop healthier attitudes, stronger coping skills and positive strategies to address the concerns that led to treatment. Care is guided by the Together Facing the Challenge (TFTC) evidence-based treatment model. It emphasizes relationship-building, skill development, and strength-based approaches. A multidisciplinary treatment team works closely with each youth and his family to design an individualized treatment plan focused on specific needs and goals, with progress reviewed regularly. Weekly programming includes anger management groups, strategies groups, independent living skills training, drug and alcohol education, group therapy and individualized assignments. Individual therapy and family therapy may be offered on an individualized basis according to the treatment team's recommendations. The program also emphasizes the role of therapeutic groups. This environment helps youth learn and practice respect for self and others, develop healthy relationships, set and meet expectations, and take responsibility for creating a safe and supportive community.

Outpatient mental health counseling center providing 100% free services to eligible Veterans, active duty service members, and their families. Part of the US Department of Veterans Affairs but separate from the VA Medical Centers, with separate and confidential medical records. Offering individual, group, and couples counseling both in Rapid City and at a number of Community Access Points in the Black Hills. Also, a referral service and can point Veterans in the right direction for any information they may need in terms of accessing benefits, enrolling in VA health care, etc. Specialty is helping Veterans readjust successfully into civilian life, and provides counseling for a wide range of problems, from mental health issues like PTSD, depression, and anxiety to life stressor issues such as work-related or relationship difficulties. Provides free bereavement counseling for loved ones of service members who died while on active duty or Veterans who died by suicide.

A walk-in clinic offering patients to receive care for common, non-life threatening medical conditions from primary care providers. On-site lab tests and imaging exams are available.

Investigates child abuse and neglect cases by working with the State's Attorney, law enforcement, and community resources to determine extent of alleged child abuse or neglect. Other services include adoptions, licensing of foster homes, assessment services, case management, and intensive family services. Sanford Children's Parenting Services offers a variety of services and resources to get answers to everyday parenting questions. Services are geared toward enhancing the development, health, and safety of children in eastern South Dakota. Provides empowerment to families, caregivers, and people who directly impact the lives of children. Also provides parenting resources through New Baby in Home services, Getting to Know Baby Better resources, and parent navigation to support parents on the parenting journey.
