Services include emergency room, ambulance, cardiac rehabilitation, chronic care management, gastroscopy, intensive/coronary care unit, mammograms, pulmonary rehab, physical, speech, and occupational therapies, and a sleep lab. Provides monthly services for blood pressure/blood sugars, walk-in immunizations, and foot care clinic. Also includes outreach clinics/telemedicine services for a variety of health needs.

In-home case management for children, youth, and/or families that are at risk of system involvement (child welfare, juvenile justice, criminal justice, etc.). Assists caregivers strengthen their families and prevent child abuse and neglect through education, skill development and therapeutic crisis management.

ICAP Housing Services provides the following programs: Rapid Rehousing and Homeless Prevention Programs: aims to help two specific groups - persons who are at great risk of homelessness and those who have become homeless. Services that can be provided include case management, security deposit and rent assistance. These services are aimed at helping participants to retain their housing, therefore avoiding homelessness, or to re-enter appropriate permanent housing as quickly as possible. Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Homeless Rent and Deposit: Helping homeless households get into housing. Must provide steady and reliable source of income. Applicant must agree to case management services.

Pharmacy services include a wide variety of brand and generic prescription medications, private-label, over-the-counter medications, vaccinations, home medical equipment, email and text message refill reminders, mail services, and pharmacist counseling. This is also a medication disposal site.

Offers skilled nursing care for either short-term rehabilitation or extended long-term care needs. Provides daily activities, physical, occupational, speech and respiratory therapy, and 24-hour a day nursing care. Accepts Medicare and Medicaid.

Staff has been trained and tested in the care of dementia patients and are supervised and evaluated regularly by Registered Nurses. Services offered are home health, companionship, homemaking assistance, respite care, pediatrics, home care & support services, medical staffing, and long term care assessments.

Horizon Health - Yankton Medical offers Medical care (behavioral health, geriatric, chronic care management, DOT physicals, physical exams, family medicine, maternal health, family planning, sexually-transmitted disease testing, nursing, health education, immunizations, preventative services, well child exams, acute care, immunizations, lab/x-ray, and women's health). Also offers Certified Application Counselors for the Health Insurance Marketplace. Horizon Health - Yankton Dental offers dental care services include cleanings, dental exams, bridges, crowns, dentures, digital x-rays, emergencies, extractions, fillings, mouth guards, partials, root canals, sealants and spacers.

Accelerate Center for Intensive Early Intervention serves: Kids ages 2-6 (with speech and occupational therapies; addressing areas such as communication, social interaction, regulation and coping, peer interactions, motor and daily living skills) Kids lagging behind developmentally with or without a diagnosis Kids with autism Kids with Sensory Processing Disorder Kids with other genetic or developmental disabilities Skills are addressed in small groups of 3-4 children in 2 hour sessions, 5 days per week. The daytime program provides the following services: case management for connection with needed resources, access to computers and a phone for housing and job searches, showers, laundry, clothing bank, hot coffee, lunch, and temporary mail address services.

Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

Behavioral Health Services available from licensed substance abuse and mental health counselors. ​Services include: chemical dependency assessments and Level 1.0 counseling, individual, couples, and family behavioral health counseling, Medication Assisted Therapy (MAT) for patients recovering from substance use disorders, EMDR (Eye Movement Desensitization Reprocessing) Therapy, and psychiatry team availability. Transportation is available for current patients if needed.

Mental health therapy for anxiety, depression, stress, life changes, and relationship issues through individual counseling, trauma informed therapy, and cognitive behavioral therapy. Sessions can be provided in person or via telehealth.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

K-5 (Kindergarten through 5th Grade) provides care and programming outside of classroom hours in several locations in Sioux Falls, Brandon and Harrisburg. Activities are designed to meet each age level's academic, social and emotional needs. Signature programs offer additional options for character and leadership, health and wellness, sports and recreation and Education/STEM/Arts focus. Scholarships are available for families in need.

First request for food assistance will be handled on a current needs basis following a short interview process with the coordinators of the food bank, the Pastor of Little White Church, or office secretary. An application for assistance will need to be completed providing contact information. A confidential record of usage will be maintained in the church office. Users of the Food Pantry will be provided bags of food for 3 consecutive weeks. This amount of food should help a struggling family or individual to get back on solid ground. Following the first 3 weeks, users of the Food Pantry can access usage of the Food Pantry every other week receiving 2 bags of food per person according to posted limitations. If there is a change in an individual's circumstances additional assistance may be considered at the direction of the coordinators of the food bank, the Pastor of Little White Church, or office secretary.

Options include Birthday Parties, Horseback Riding, Vacation Camps, Outdoor Education, Teambuilding, Weddings, Corporate Outings and Special Events.

Works with teens and adults to process through the hurts of the past and focus on the possibilities in the future. Offers counseling for individuals, family, marriage, couples, adolescent/youth, parents, LGBTQ, grief, substance use, hoarding, spouse/intimate partner abuse, and child abuse survivor for adults. Cognitive behavioral therapy (CBT), custody evaluations, and telemental health counseling are available.

Individual, couples, and family counseling for all ages regarding issues of depression, anxiety, grief and loss, recovery, stress, crisis, relationship issues, anger, forgiveness, communication, self-concept, learning disabilities, attention deficit / hyperactive disorder (ADD/ADHD), parenting, and spiritual direction. Also, provides tele-counseling (via phone). Preferred that inquiring clients attend the intake appointment in person.

Providing confidential and no-cost early pregnancy services including pregnancy testing and limited obstetrical ultrasound. Other services include Confidential peer counseling, emotional support, information and education on pregnancy options, post-abortion help, pregnancy and parenting classes and referrals for community resources.

Provides mental health and substance use counseling services including but not limited to the following topics: alcohol abuse, anger management, anxiety, behavioral issues, borderline personality, career counseling, child or adolescent, chronic impulsivity, chronic relapse, codependency, coping skills, depression, drug abuse, dual diagnosis, family conflict, gambling, grief, internet addiction, life transition, parenting, peer relationships, self esteem, stress, substance abuse, trauma, post traumatic stress disorder (PTSD), video game addiction, impulse control disorders, and personality disorders. Telehealth session are available. First session is preferred to be in person.
