Founded on the belief that everyone deserves a home. Pathways offers four different shelter programs:
  • Emergency Shelter for Families:
8 rooms designated for households with children. Families will receive 30 days for free in a 12-month period. After 30 days, rent will be charged for extended stay.
  • Tier 2 Services:
2 rooms designated for men and women seeking a more permanent residence and access to more services. To qualify for this program, individuals must be employed or be able to pay rent at the start. Total of 4 beds for women and 4 beds for men.
  • Re-Entry Program:
Partnership with South Dakota Department of Corrections; provides 8 beds for men who are needing to re-enter the community. These individuals must follow the same guidelines for admission as other clients. Young Adult Transition Program: Provides transitional living and case management for young adults 18 to 24 years old who are experiencing homelessness. 4 beds are available for this purpose and can help people within the age range struggling in the community. Contact Staff for more details. Through the Emergency Solutions Grant, Pathways offers two programs:
  • Homeless Prevention
For individuals and families who are unable to pay their rent or utilities and will otherwise be displaced from their residence. Call staff to learn more about eligibility requirements.
  • Rapid Re-housing
For individuals who have found potential housing but may not yet have the funds to secure that housing. For those who are eligible, Pathways may be able to pay first month's rent, rental deposit and utility deposit. Call staff to learn more about eligibility requirements. Funding through the Security Deposit Assistance Program allows Pathways to operate a program solely to assist with security deposits.

A national program that promotes school readiness by enhancing the social and cognitive development of children through the provision of educational, health, nutritional, social and other services to enrolled children and their families. Services include head start and early head start part and full day, school year, and full year options, breakfast, lunch and snack provided, transportation is available for those with need, cultural and language curriculum for the Lakota/Dakota Language, developmental screenings, and services for prenatal mothers.

Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Mental Health services include individual, couples and family counseling, 24-hour QMHP services, psychiatric evaluations and medication management, school consultations, and employee assistance programs (EAP). Alcohol and Drug Program treatment approach is based on the 12-step program of Alcoholics Anonymous and reality therapy. Services offered include individual counseling, chemical assessments, DUI education program, outpatient treatment, prevention education program, and a relapse prevention group. Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A client centered, client directed form of treatment, the program consists of services designed to restore the disabled person to a level of functioning which permits a useful, meaningful life.

If the family income is below 140%, the children may be eligible with or without private health insurance. If there is insurance, CHIP will pay the deductibles, co-insurance, and possibly some medical services not covered by the private insurance. ​Medicaid: Health care coverage for people who qualify. To be eligible for Medicaid, individuals must fall into a coverage group and meet certain financial and non-financial eligibility criteria. Investigates child abuse and neglect cases by working with the State's Attorney, law enforcement, and community resources to determine extent of alleged child abuse or neglect. Other services include adoptions, licensing of foster homes, assessment services, case management, and intensive family services. Provides Public Health Nursing and Maternal Child Healthcare Services through various programs. General public health services include professional nursing and nutrition information and guidance on health, safety, and nutrition for individuals, families, and communities. Services such as influenza vaccinations and screenings for blood pressure, hearing, vision, and hemoglobin are available at a fee. Maternal Child Healthcare provides Infant Sleep Safety services, immunizations, and pregnancy support services. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.

The Health Cooperative is here to partner with patients to stay healthy and meet their health goals. An inter-professional team is available to offer early evaluations of adult health concerns. Also provides personalized self-care education that will assist patients in making healthier choices. This is not a replacement for a primary care doctor. Works alongside patients and their doctors to improve patient health. Team consists of registered nurses, a pharmacist, an occupational therapist, a physical therapist and a dietitian.
