Medical Prescription Assistance - Financial assistance may be offered for those with a medical prescription. Medical Equipment Lending - Available for those in need of bathtub seats, shower and tub rails. Items are based upon availability and may vary. To qualify, a picture ID is required along with a current lease or rental agreement. Clothing Voucher: Clothing assistance is available every 3 months per individual from thrift store. To qualify, a picture ID is required along with verification for additional individuals in the household such as medical cards, birth certificates, etc. Items are based upon availability and may vary. Pathway of Hope: Long term case management program to help individuals and families move beyond crisis into stability and self sufficiency. To qualify, individuals are required to meet with a caseworker to design a plan to meet goals, needs and objectives (goal setting, financial counseling, assistance getting healthcare, educational opportunities, etc.).
