Coteau des Prairies (CDP) Health Care System is a 25-bed, critical access acute care, community non-profit hospital serving approximately 21,000 residents in the Glacial Lakes Region in north east South Dakota, and west central Minnesota. CDP Health Care System owns and operates the attached certified rural health clinic in Sisseton, and the Rosholt Clinic in Rosholt, South Dakota.​ Services available at Coteau des Prairies Hospital and Clinic include In-patient and out-patient surgery, Swing Beds, Emergency Room, Cardiac Rehab, Laboratory Services, Radiology, Mammography, Therapy Services, Fitness Center, Medication disposal, Diabetic Education, Telehealth, Medical-Assisted Treatment (MAT), Nutrition Services, and Women's Health. Provides assistance to individuals with varying abilities to live as independently as possible through home care services, companion care, personal care, and in-home nursing care.

Provides long-term care nursing-facility services for those who can no longer remain in their home and short-term rehab services for those recovering from an illness or surgery. These services include physical, occupational, and speech therapy, 24-hour nursing care, dietary services, social activities, and more.

The Health Cooperative is here to partner with patients to stay healthy and meet their health goals. An inter-professional team is available to offer early evaluations of adult health concerns. Also provides personalized self-care education that will assist patients in making healthier choices. This is not a replacement for a primary care doctor. Works alongside patients and their doctors to improve patient health. Team consists of registered nurses, a pharmacist, an occupational therapist, a physical therapist and a dietitian.

Services offered include nursing services (e.g. medication changes, health assessment, monitoring vitals, etc.), therapy (physical, occupational, and speech), personal care, homemaking and respite care. Offers a Foot Care Clinic on the 2nd and 4th Tuesday of each month by appointment.

Home Health may be considered for people who are recovering from an illness, injury, or surgery, undergoing treatment, people with disabilities, and those who are chronically ill or terminally ill. Services offered are skilled nursing care, home health aides for personal care, rehabilitation therapy (physical therapy, occupational therapy, and speech therapy), social services, patient/family education, medication monitoring, and nutrition consultation.

Services are provided within the patient's home. Includes non-skilled nursing care (e.g. foot care, general assessment, medication management, colostomy care, etc.), personal care (e.g. bathing, hair care, oral/mouth care, exercises, and medication reminders), and housekeeping (e.g. vacuuming, dusting, kitchen/refrigerator cleaning, bathroom cleaning, laundry, etc.). Provides skilled in-home physical, occupational, and speech therapy, as well as social work.

Home Health: Offered skilled home health services including nurses, physical therapy, occupational therapy, speech therapy, and home health aides providing care, support and education to allow patients to manage health conditions at home. Hospice: Offers a multidisciplinary team that addresses the patient and their family addressing physical, psychosocial, spiritual, and bereavement needs. Services include clinician on-call 24/7, pain management, guidance with Advanced Care Planning, forms, and financial care needs, provide medications, durable medical equipment, and supplies needed to treat life limiting illness, support and teach family/caregivers how to care for the patient, and grief/bereavement counseling for up to 13 months after the patient has passed away.

Offers skilled nursing care for either short-term rehabilitation or extended long-term care needs. Provides daily activities, physical, occupational, speech and respiratory therapy, and 24-hour a day nursing care. Accepts Medicare and Medicaid.

They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.
