The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. CARE Club - Program for individuals with a mental health disability (i.e. schizophrenia, bipolar disorder, anxiety and borderline personality) who live in the community. CARE club provides support, assistance, interaction with others and the opportunity to improve consumers existing skills and develop new coping skills. Children's Services - Provides child centered therapeutic services in the home or office. This program provides mental health services to children with Serious Emotional Disturbance (SED). The parents, families, and surrogate families of children with SED participate in all aspects of evaluation, planning and delivery of services. Works closely with child-serving agencies and programs.

Case Management and IMPACT Programs, designed to help clients with severe and persistent mental illness, services include case management and continued care, medication management with psychiatrist, crisis intervention assistance, and psychiatric rehabilitation which seeks to provide program services that assist clients in developing skills and manage resources needed to allow them to be successful in the community. A full continuum of mental health and treatment for substance use disorders. Services include assessments, individual and family counseling, intensive outpatient substance use disorder treatment, residential treatment for pregnant and parenting women who face substance use disorders, transitional care and aftercare counseling for adults and adolescents, trauma-focused treatment for children/adolescents and their families impacted by traumatic events.
