Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Community Health Representatives | Oglala Sioux Tribe

Provides direct patient health care services including: promoting education and awareness of proper diet, exercise and health care practices; providing health care to patients with diabetes complications; education on reducing/preventing disease and accidents; providing transportation to health care facilities for patients; assisting IHS in providing baby and well-child immunizations checks; informing proper agencies of any violations and conducting follow-ups; gerontology for the elderly population; reducing rate of teenage pregnancy, STD's, with awareness and prevention practices; conducting Wellness, Health and Nutrition Awareness workshops/clinics; providing direct health care services to physically impaired individuals; providing medical transportation by means of the Transit Program; providing community health worker services; and providing animal care services to canine and feline species.

Services include Walk-in Urgent Visits (for both medical and dental), Schedule and same day appointments, Family Practice and Internal Medicine Primary Care for adults, Dental Care for all ages (including teeth cleanings), Sexual and Reproductive Health Services, On-site lab and X-Rays, On-Site Pharmacy, and After-Hours On-Call Provider 365 days a year. Primary and Pediatric Medical Clinic: Most routine medical services and several specialized fields including: physicals, immunizations, preventative medicine, chronic disease management, and counseling. Well child checkups, vaccinations, general pediatric health care. Mental Health: Offers outpatient counseling and psychiatric services, to both children and adults. Case management is offered to adults and families. Dental Clinic: Provides emergency, preventive, and oral health care. Accepts Medicaid, Private Insurance, Medicare, free of charge for the homeless and on a sliding fee scale for uninsured. One Heart Clinic: Providing primary family care to patients with a focus on patients with a primary language of Spanish. Pharmacy: Convenient, on-site location, CHCBH patients are able to fill most of their medications the same day as appointment.

Adoption and Support Services | All About U Adoptions

For anyone facing an unplanned pregnancy, All About U provides crisis pregnancy counseling, help finding emotional support, help finding necessary resources, emotion support throughout adoption and beyond, if adoption is chosen, emergency transitional housing available, and general case management for resources: connection to prenatal care, housing, food, clothing, addiction, and more. For adoptive families, All About U provides training to help them through the adoption process and prepare them to be an adoptive family, home studies, and on-going support for adoptive families.
