Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Substance use evaluation, assessment, and treatment is also provided. Walk in assessments and telehealth services are available during normal business hours at the Aberdeen location. Offers limited food assistance as part of Feeding South Dakota's mini-food-pantry system. Service is limited to one pre-packed bag of food per household per month. Supplemental Nutritional Assistance Program (SNAP) is a supplemental food program that enables eligible households to obtain a more nutritious diet. Eligibility is determined by income, resources, and household size. Lost, stolen, or damaged EBT (SNAP) cards must call customer service to report the loss of your card. You will need to enter the card number. If you do not have the card, stay on the line for a second prompt and press 2. Reporting an EBT card as lost, stolen, or damaged will result in that card being declined.
