An independent entity working alongside and in cooperation with Cornerstone Prison Church (prison congregation in Sioux Falls), Livingstone Prison Church (prison congregation in Springfield), and Metal Craft Industries.

Horizon Health - Alcester Medical is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Also offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT).

Provides long-term nursing facility care and short-term inpatient rehabilitation care with 24/7 nursing care and daily assistance. Also provides community-based independent living options. Birthparent Counseling Program provides options counseling to assist birth parents who are in an unintended/crisis pregnancy in reaching decisions regarding their futures and the future of the child. Services to birth parents are free of charge. Services are available anywhere in South Dakota, LSS can travel to meet with birth parents. ​Parenting and adoption options are explored: If parenting is the decision, the counselor will assist in setting up community resources. If adoption is the decision, the birth parent can choose from adoptive parent profiles, including pictures and information about the family. The birthparent also can choose what degree of openness they want in the adoption plan. LSS does provide both open and closed/confidential adoption. The purpose of this program is to assist birthparents in reaching a permanent plan for themselves and their child. Adoption Program provides options for those desiring to pursue having a family, including Infant Domestic Program and International Adoption Program. Training and Home Study Services are provided prior to placement of a child.

Pharmacy services include immunizations, prescription refills, health screenings, and a medication disposal drop box. - Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste.

Provides primary LGBTQ+ health care for people up to the age of 21. This includes routine check-ups, sexually transmitted/HIV/pregnancy tests, and sexual health education. An in-house pharmacy is available as well as mental health counseling services. Dr. Amanda Diehl has more than 20 years of experience and trained on LGBTQ+ health care through the Fenway Institute's National LGBT Health Education Center. In addition, the entire staff has been trained to provide an affirming atmosphere for the LGBTQ+ population.

4-week class for parents with children ages birth - 3 years old. Parents will learn about topics such as social-emotional growth, early brain development, temperament, and guidance. This class is usually an online, virtual, class.

The Medicaid Fraud Control Unit investigates and prosecutes acts of abuse, neglect or exploitation perpetrated upon any resident in a health care facility that receives federal funding. The Medicaid Fraud Control Unit also investigates and prosecutes fraud and abuse committed by physicians, hospitals, laboratories, pharmacists, dentists, ambulance and taxi services, sellers of durable medical equipment, and other service providers.

Providing free lunch for children 18 years old or younger. Meals on Wheels Western South Dakota are funding lunch at no charge for seniors (60+) accompanying their grandchildren. ​The Black Hills Area Community Foundation will fund lunch for adults (19-59 years old) accompanying their children. After-school care for boys and girls ages 10 - 18 meeting intensive outpatient guidelines for addiction recovery treatment. Services offered include drug and alcohol treatment needs assessment, intensive outpatient group program, Continued Care group program, Individual therapy, and the Learning Above and Beyond (LAB) afterschool prevention based program. ​Telehealth options available.

ICAP Housing Services provides the following programs: Rapid Rehousing and Homeless Prevention Programs: aims to help two specific groups - persons who are at great risk of homelessness and those who have become homeless. Services that can be provided include case management, security deposit and rent assistance. These services are aimed at helping participants to retain their housing, therefore avoiding homelessness, or to re-enter appropriate permanent housing as quickly as possible. Housing Repair / Rehabilitation: Provides a zero-interest, five-year decreasing balance loan for eligible homeowners to be used for accessibility modifications for persons with disabilities, in addition to making necessary repairs for health, safety and code compliance. Self-Help Housing Rehabilitation: Eligible participants will work with program staff to identify their specific needs and to create a work plan. The Program Coordinator will find the best funding sources available to the homeowners and assist them throughout the process. Weatherization and Furnace Replacement Program: Aimed at making housing for low-income persons more energy-efficient and less costly for the residents. The residence is audited and a list of needs is developed. The program provides the materials and pays contractors for the labor to weatherize the home (caulk cracks, insulate, weather-strip, fix windows, etc.). Furnaces can be repaired or replaced, if not repairable, through the program. In addition, some inefficient/high-cost non-operating appliances (range / refrigerator / water heater) can also be replaced in certain circumstances. Homeless Rent and Deposit: Helping homeless households get into housing. Must provide steady and reliable source of income. Applicant must agree to case management services.

Specialties include mental health screenings, divorce, grief, cognitive behavioral therapy (CBT), dialectical behavior therapy (DBT), adolescent/youth, terminal illness, LGBTQ/gender identity, parenting, hoarding, spouse/intimate partner abuse, adult survivors of child abuse. Also offers services via telehealth.

Provides quality home health care to residents of the Sioux Empire, Vermillion, and Chamberlain communities. Services offered include care for heart conditions, complex wound care, diabetes education and assessment, disease and symptom management, education on a newly diagnosed medical condition, home health aide, home infusion therapy education, home safety education, medication education, occupational therapy, pain management, pediatric care, physical therapy, postsurgical care, skilled nursing, speech therapy, and rehabilitation therapy while recovering from an orthopedic condition or hospital stay. They are most appropriate for individuals recently discharged from the hospital and are not fully recovered such as after surgery or new diagnosis, individuals recently diagnosed with an illness and needing education, and individuals who have a chronic condition that needs frequent clinical assessment such as diabetes, kidney, stroke, heart or lung disease.
