Provides services and supports to children and adults with developmental disabilities by providing support, skills, and encouragement needed to achieve hopeful and successful futures. Services provided to program participants include special education, adult day programs, vocational programs, home-based instruction, residential living facilities, consultations, psychiatric services, medication management, counseling, and transition program.

The Department of Human Services Guardianship Program provides guardianship and/or conservatorship services as a last resort to qualified individuals age 18 or older with an intellectual or developmental disability as defined in SDCL 27B-1-18 or qualified individuals in the Long Terms Services and Supports program. The South Dakota Department of Human Services may be appointed as a guardian, a conservator, or both, for adults with developmental disabilities under its care or to whom it is providing services or financial assistance, but only if there is no individual, nonprofit corporation, bank or trust company, or other public agency that is qualified to serve, as defined in SDCL 29A-5-110, or qualified individuals in the Long Terms Services and Supports program. The department also provides financial assistance for qualified individuals who are willing and qualified to serve as a guardian and/or conservator of up to $1,500.00 for the establishment of a guardianship or conservatorship through the Guardianship Establishment Program. Limited amount of funds for this program are available each year on a first come first served basis for qualified individuals.

Made up of 25 members appointed by the Governor. The Council receives federal dollars that it then grants to other community agencies for projects benefiting people with developmental disabilities (DD), projects include funding for individuals with DD and their family members to attend workshops and conferences, Partners in Policy-Making, Youth Leadership Forum, SD Advocates for Change, and Information and Referrals.

CHOICES Residential Services program serves adolescents and adults who have intellectual and developmental disabilities and behavioral/emotional or psychiatric disorders. Provides a continuum of rehabilitation services for people with short- and long-term conditions, injuries, and disabilities. The goal is to improve health outcomes, make it easier for clients and families to get the right services for their needs, and reduce costs by preventing complications. Programs include: ​Brain Injury Rehabilitation - Helping patients learn new skills to help manage many of the symptoms of brain injury. ​Cancer rehabilitation - Addresses needs at each stage of treatment, helping patients cope with the effects of cancer and its treatment. ​Chronic pain services - Comprehensive outpatient pain management services. ​Comprehensive inpatient rehabilitation - Hospital-based rehabilitation program for those who require close medical monitoring and management of medical conditions. ​Comprehensive stroke rehabilitation program - Evidence-based treatments support individuals who have had a stroke as they regain daily living skills and become as independent as possible. ​Courage Kenny Kids - Offering physical and occupational therapy and speech-language pathology. ​Courage Kenny Spine Centers - Provides specialized, nonsurgical treatment for low back and neck pain. ​Spinal Cord Rehabilitation - Offers inpatient and outpatient services for persons with a spinal cord injury. ​Occupational/physical therapy and speech-language pathology - Providing clinic based outpatient rehabilitation therapy.

Locates children that are new born to 5 years of age who are suspected of having developmental delays or mental and/or physical disabilities in order to provide early intervention services. Services include screening children for developmental delays, referring children requiring further evaluation to the appropriate agencies, and providing parents with training, educational materials, and monetary and transportation assistance. This program also hosts parent support group meetings, workshops, and health fairs.

Black Hills Works is a community support provider that offers an array of programs to adults with intellectual and developmental disabilities. Services include residential, employment, volunteer and community services, specialty services, and art expression.

Sesdac provides services and supports to individuals with intellectual and developmental disabilities. Services include day services, transition services, employment supports, residential services, and educational services.

Provides adults with intellectual and developmental disabilities with hands-on experience in advanced agricultural practices while nurturing essential job and life skills. Participants engage in every aspect of farming including planting, harvesting, managing customer relationships, preserving food, and caring for livestock. The program also supports teamwork, problem-solving, communication, personal growth, and community involvement while empowering participants to explore and consider future careers in agriculture or related fields.

Helps families with temporary relief care for children or adults with special needs. The child or adult must live with a parent or family member and meet one of the eligibility categories below. There is no income limit. An adult or child eligible for family support waiver services is not eligible for respite services. Eligibility Categories include: Developmental disability (child or adult); Developmental delay (birth to age 3); Serious emotional disturbance (child); Severe and persistent mental illness (adult); Chronic medical condition (child); Traumatic brain injury (child or adult); Adopted Child (with or without a disability).

Provides housing and community-based support services for individuals with intellectual and developmental disabilities. These include: Host Homes are available for adults with intellectual and developmental disabilities where they get matched with a shared living provider and move into the provider home. The shared living provider is responsible to provide supports and services that foster independence. ​Transitions Home is a home that is staffed 24/7 and operated like a group home. It is used as a transition home while individuals are matching with their potential shared living and host home providers.
