Services include program education and support and personal attendant services. - MONEY FOLLOWS THE PERSON - Providing options for older adults and/or people with disabilities living in nursing homes, hospitals, or long-term care facilities to enable them to successfully return to their homes and communities. Helps individuals identify barriers to living on their own, helps them find a place to live that meets their needs, and helps them find the services and supports they need to continue living independently.

The Council receives federal dollars that it then grants to other community agencies for projects benefiting people with developmental disabilities (DD), projects include funding for individuals with DD and their family members to attend workshops and conferences, Partners in Policy-Making, Youth Leadership Forum, SD Advocates for Change, and Information and Referrals.

Focuses on empowering people with disabilities to live, work, and participate fully in their communities. Provides training, technical assistance, education, research, and services to promote best practices and build statewide and local community capacity. The team builds meaningful relationships with individuals with disabilities, their families, and the people they interact and work with on a day-to-day basis. Advocates for increased self-determination, independence, productivity, and a healthy and satisfying quality of life for self-advocates and the community at large. There are many opportunities for people to participate in and explore a variety of meaningful activities throughout their day. They do this in many ways, and all services are driven by each person's own goals, dreams and needs. Some areas of focus are: - Accessing their community - This may include volunteering or attending activities and events in the community. - Recreational and leisure - Swimming, bowling, spending time at local parks, movies, and eating out are a few examples. - Meaningful relationships - People are encouraged to keep in touch with loved ones whether this is writing cards or emails, sending pictures, or making arrangements for visits. - Maintaining and learning new skills - People may need continued training so they can be as independent as possible. Staff support people in learning sign language and using adaptive communication systems. Other examples of trainings offered are cooking classes, self help and social skills, computer classes and money management. - Self advocacy - People are provided training in many areas of safety and welfare. Some topics include stranger danger, learning how to protect themselves from Abuse, Neglect and Exploitation, and learning what to do for severe weather or fire. People also learn what to expect at doctor's appointments to help them overcome anxiety and fears.

All services are provided in the home. Head Start Centers offer a comprehensive, no cost early learning and support program for children 3 - 5. Residents of Corson County are served through the Lemmon Head Start Center.

Provides services, education, outreach, and advocacy so that people living with autism and other disabilities can live, learn, work and play in our communities. Primary services include medical rehabilitation, job training and employment, child care, adult and senior services, camping and recreation.

The Family Support 360 Program (FS 360) provides people and their families with services and supports to meet their unique needs. The FS 360 Program is self-directed, which means the person and their family choose the services, the staff who provide them, and a schedule based on their needs. FS 360 helps people and their family receive state funded services and other community services, together with natural supports (extended family members, friends, neighbors, church congregations, and community organizations). Each person receiving FS 360 services chooses a Family Support Service Coordinator who will help you find state funded services, community services, and natural supports. Once these services and supports are found, the Family Support Service Coordinator will help control a budget for the services. Most importantly, your Family Support Service Coordinator will advocate for you and your family. Services & Supports include Service Coordination, Environmental Accessibility Adaptive Equipment, Vehicle Modifications, Companion Services, Supported Employment, Personal Care, Respite Care, Special Medical Adaptive Equipment and Supplies, Nutritional Supplements, and Specialized Therapies.

Supports individuals in the community with developmental delays by providing residential services, employment services, day programming and nursing.

Provides support to families with children birth to 3 years with identifying needs and accessing community services. Services include: audiology, assistive technology, family training, counseling and home visits, health service, medical services for evaluation, nursing, nutrition, occupational therapy, physical therapy, psychological services, social work services, service coordination, special instruction, speech and language, transportation for services, vision, and other services identified by the team.

Provides a wide range of services and support including residential, work and specialized services. Also provides individualized service facilitation to those served. Firmly dedicated to the provision of quality, outcome-based services to persons with developmental disabilities. - Residential options living at home, supported living, independent living - Work & employment community employment, supported community work-sites, agency provided work opportunities, supported self-employment - Specialized services medical services, behavioral services, communication services, nutritional services, physical and occupational therapy

The Department of Human Services Guardianship Program provides guardianship and/or conservatorship services as a last resort to qualified individuals age 18 or older with an intellectual or developmental disability as defined in SDCL 27B-1-18 or qualified individuals in the Long Terms Services and Supports program. The South Dakota Department of Human Services may be appointed as a guardian, a conservator, or both, for adults with developmental disabilities under its care or to whom it is providing services or financial assistance, but only if there is no individual, nonprofit corporation, bank or trust company, or other public agency that is qualified to serve, as defined in SDCL 29A-5-110, or qualified individuals in the Long Terms Services and Supports program. The department also provides financial assistance for qualified individuals who are willing and qualified to serve as a guardian and/or conservator of up to $1,500.00 for the establishment of a guardianship or conservatorship through the Guardianship Establishment Program. Limited amount of funds for this program are available each year on a first come first served basis for qualified individuals.

Sesdac provides services and supports to individuals with intellectual and developmental disabilities. Services include Day Services, Transition Services, Employment Supports, Residential Services, and Educational Services.

RHD's Shared Living Program has three service lines. - Host Home: Adults with intellectual and developmental disabilities get matched with a shared living provider and move into the provider home. The shared living provider is responsible to provide supports and services that foster independence. - Day Services: Day service location in Sioux Falls that operates 8:30am - 3:30pm, Monday - Friday. - Transition Home: This home is staffed 24/7 and operated like a group home. It is used to house individuals while they are matching with their potential shared living providers.
