Provides medical, mental health, and complementary services in one location, by providers that collaborate care, to ensure the best treatment outcomes for clients. Services are available to all people of all ages seeking healing from mental and physical injury to improve health and wellness. Services provided are mental health counseling for individuals, couples, and families. The Wellness Collective also provides functional medicine, nutrition therapy, massage, yoga, and psychiatry services. Telehealth is offered for individuals physically located in South Dakota, and psychiatry services are also available via telehealth for individuals located in ID, IA, KS, MI, MN, ND, and WY.

The South Dakota Birth to Three Early Intervention Program serves children from birth to 36 months with developmental delays or disabilities and their families. The Family Support 360 Program (FS 360) provides people and their families with services and supports to meet their unique needs. The FS 360 Program is self-directed, which means the person and their family choose the services, the staff who provide them, and a schedule based on their needs. FS 360 helps people and their family receive state funded services and other community services, together with natural supports (extended family members, friends, neighbors, church congregations, and community organizations). Each person receiving FS 360 services chooses a Family Support Service Coordinator who will help you find state funded services, community services, and natural supports. Once these services and supports are found, the Family Support Service Coordinator will help control a budget for the services. Most importantly, your Family Support Service Coordinator will advocate for you and your family. Services & Supports include Service Coordination, Environmental Accessibility Adaptive Equipment, Vehicle Modifications, Companion Services, Supported Employment, Personal Care, Respite Care, Special Medical Adaptive Equipment and Supplies, Nutritional Supplements, and Specialized Therapies.

The Department of Human Services Guardianship Program provides guardianship and/or conservatorship services as a last resort to qualified individuals age 18 or older with an intellectual or developmental disability as defined in SDCL 27B-1-18 or qualified individuals in the Long Terms Services and Supports program. The South Dakota Department of Human Services may be appointed as a guardian, a conservator, or both, for adults with developmental disabilities under its care or to whom it is providing services or financial assistance, but only if there is no individual, nonprofit corporation, bank or trust company, or other public agency that is qualified to serve, as defined in SDCL 29A-5-110, or qualified individuals in the Long Terms Services and Supports program. The department also provides financial assistance for qualified individuals who are willing and qualified to serve as a guardian and/or conservator of up to $1,500.00 for the establishment of a guardianship or conservatorship through the Guardianship Establishment Program. Limited amount of funds for this program are available each year on a first come first served basis for qualified individuals.

Providing personalized applied behavior analysis therapy to those who have an autism spectrum disorder diagnosis, developmental delay, anxiety, attention deficit hyperactivity disorder, obsessive compulsive disorder, depression, genetic disorders, sleep disorders, or other conditions. Services focus on skills like language, social interaction, daily living, and behavior replacement. This in-home therapy includes parent training and behavior consultations.
