The SSVF program provides homelessness prevention and rapid re-housing services to help Veteran families keep, or rapidly transition to permanent housing. For veterans and their families that meet all eligibility criteria and are enrolled with the program, the program can provide: Outreach to Veterans Case management

Horizon Health - Isabel is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Horizon Health - Isabel is also a partner with the Department of Veterans Affairs (VA) Community Care Program.

The American Red Cross works in partnership with Military Aid Societies to provide financial assistance to eligible applicants. Assistance can include funds for emergency travel, burial of a loved one, emergency food and shelter, etc. The Aid Societies determine the financial assistance package that will be offered through a grant or a loan.

Behavioral health services including addictive disorder services (ADS), compensated work therapy, Residential Post Traumatic Stress (PTSD) program, Mental health intensive case management, inpatient psychiatric services, and outpatient psychiatric services. Bereavement services for family members is also available. Readjustment counseling is offered to make a successful transition from military to civilian life or after a traumatic event experienced in the military. Individual, group, couples, and family counseling is offered in addition to referral and connection to other VA or community benefits and services.

The Mental Health Service Line provides connection to outpatient mental health services, psychiatry services, local recovery coordination, care coordination, and peer support services. Also offers care for military sexual trauma, PTSD, suicide prevention and awareness outreach, and gun safety locks. Telehealth service options are available.

The Extended Care and Rehab Service Line includes the following services: Caregiver support group, spinal cord injury / disorder program, full suite of therapies, chiropractic care, integrative health services, adaptive sports, low vision and visually impaired services, inpatient community living center, pain management, whole health services, community living center, hospice/palliative Care, polytrauma/traumatic brain injury (TBI), geriatrics & extended care (GEC), medical foster home, and home health care.

Provides support to veterans through financial aid, legal guidance, transportation, therapy, treatment access, disability claims assistance, and equine therapy. Offers advocacy services to address barriers veterans face. If direct help isn't available, staff will connect individuals to the appropriate resources.

Services include, but are not limited to the following areas: Service to veterans Veterans service officer - who is able to assist in obtaining benefits Advocacy programs for veterans and people with disabilities Legislation Peer counseling - for persons with physical disabilities and especially new spinal cord injuries and multiple sclerosis. Sports and recreational activities - Assists veterans that have physical disabilities with getting involved in sports activities such as bowling, lacrosse, boccia ball, fishing, skiing, trap shooting, and hunting. Also, a referral service and can point Veterans in the right direction for any information they may need in terms of accessing benefits, enrolling in VA health care, etc. Specialty is helping Veterans readjust successfully into civilian life, and provides counseling for a wide range of problems, from mental health issues like PTSD, depression, and anxiety to life stressor issues such as work-related or relationship difficulties. Provides free bereavement counseling for loved ones of service members who died while on active duty or Veterans who died by suicide.

The VA Surgery / Specialty Care Service Line includes the following services: Orthopedics, ENT, Gynecology, Urology, general surgery, Opthalmology, Podiatry, Audiology, Optometry, Dental, GI Endoscopy, and Proctology.

Designed to accommodate 14 veterans each day. Services include shelter, 3 meals a day, case management, helping replacing documents (birth certificates and IDs), benefit review, and employment search assistance.

Services provides for Hyde County including public transportation is available in the Highmore area. ADA accessible vehicles are available to passengers with special needs. Medical appointments pre-scheduled between 10:00am - 1:00 pm CT 9:00am - 12:00pm MT before date of appointment. Trips to Rapid City or Sioux Falls will be coordinated with River Cities Public Transit out of Pierre. (This may affect pick-up times). All out of town medical trips are on a 1st come 1st serve basis and are based on driver availability. Non-medical out of town trips available with 48 hour notice. Serving the paratransit needs of Sioux Falls by working with the City of Sioux Falls, DakotAbilities, and Lifescape. Contracted with DakotAbilities, Lifescape, the Sioux Falls School District and works through the City of Sioux Falls to help provide paratransit services to the Sioux Falls area. ​Accepts ride requests for any transportation needs in the Sioux Empire area and beyond.
