The SSVF program provides homelessness prevention and rapid re-housing services to help Veteran families keep, or rapidly transition to permanent housing. For veterans and their families that meet all eligibility criteria and are enrolled with the program, the program can provide: Outreach to Veterans Case management - Financial assistance (for securing/preserving housing) Utility assistance for water, lights, and heating for current, past due, disconnection notices, previous address, and deposits Rental assistance Security deposits - Other financial assistance Child care costs Moving costs Transportation expense Help applying for Department of Veterans Affairs assistance and other benefits Help connecting Veterans with other community resources and services

Horizon Health - Isabel is full-service medical clinic offering the following services - preventative screenings, acute care, chronic care management, well child visits, men's and women's health, immunizations, family planning, lab/x-ray, patient education, care management, DOT exams, after-hours access to care, and Certified Application Counselors for the Health Insurance Marketplace. Offers behavioral health services: medication management, counseling, case management, integrated care, addiction services, and Medication Assisted Therapy (MAT). Community-based counseling center that provide a wide range of social and psychological services, including professional readjustment counseling to eligible Veterans, active duty service members, including National Guard and Reserve components, and their families. Bereavement services for family members is also available. Readjustment counseling is offered to make a successful transition from military to civilian life or after a traumatic event experienced in the military. Individual, group, couples, and family counseling is offered in addition to referral and connection to other VA or community benefits and services.

Also, a referral service and can point Veterans in the right direction for any information they may need in terms of accessing benefits, enrolling in VA health care, etc. Specialty is helping Veterans readjust successfully into civilian life, and provides counseling for a wide range of problems, from mental health issues like PTSD, depression, and anxiety to life stressor issues such as work-related or relationship difficulties. Provides free bereavement counseling for loved ones of service members who died while on active duty or Veterans who died by suicide.

TBI evaluation and education for Veterans including speech-language, neurological, occupational, physical evaluations and therapies. Psychological services and caregiver supports are available.

Provides interdisciplinary care, medical devices and products, and rehabilitative care through evidence-based treatment. Services include: audiology, blind rehabilitation, Veteran sports programs and clinics, wheelchair games, physical medicine and rehabilitation, occupational therapy, prosthetic and sensory aid services, speech pathology, chiropractic care, orthotic and prosthetic services, physical therapy, kinesiotherapy, and recreation therapy.

Offering health care services such as primary care, laboratory and pathology, mental health care, treatment in post traumatic stress disorder (PTSD), and suicide prevention.

The VA Surgery / Specialty Care Service Line includes the following services: Orthopedics, ENT, Gynecology, Urology, general surgery, Opthalmology, Podiatry, Audiology, Optometry, Dental, GI Endoscopy, and Proctology.

VA is committed to ending Veteran homelessness. No one who has served our country should ever go without a safe, stable place to call home. The HUD-VASH program exists to assist veteran's with establishing and maintaining housing. Intensive case management is provided to the veteran.

General medical and surgical, primary, nursing home, and rehabilitative care. Behavioral health services including addictive disorder services (ADS), compensated work therapy, Residential Post Traumatic Stress (PTSD) program, Mental health intensive case management, inpatient psychiatric services, and outpatient psychiatric services. Gun locks are available through the Suicide Prevention Coordinator.

Mobile Vet Center (MVC) are large mobile vehicles with space for confidential counseling and are used to provide outreach to individuals in communities that are distant from existing services. Also provides emergency services in response to national emergencies and disasters.

Provides support to veterans through financial aid, legal guidance, transportation, therapy, treatment access, disability claims assistance, and equine therapy. Offers advocacy services to address barriers veterans face. If direct help isn't available, staff will connect individuals to the appropriate resources.

The American Red Cross works in partnership with Military Aid Societies to provide financial assistance to eligible applicants. Assistance can include funds for emergency travel, burial of a loved one, emergency food and shelter, etc. The Aid Societies determine the financial assistance package that will be offered through a grant or a loan.
