Falls Community Health is a family practice medical clinic and dental clinic with emphasis on health promotion and disease prevention. The Community Health Center was established in 1979 with its goal being to remove the barriers, primarily financial, that exist and limit the access of lower income individuals or families seeking primary health care services.  ​Services for the general public include family health, lab radiology, well-child care, immunizations, family planning (including birth control, STD screening and treatment), patient education, and dental care. Services for established clinic patients include outpatient mental health and substance abuse counseling, psychiatry services, transportation assistance (transporter of last resort), and pharmacy assistance to clients of the Falls Community Health Care Clinic.

Birthparent Counseling Program provides options counseling to assist birth parents who are in an unintended/crisis pregnancy in reaching decisions regarding their futures and the future of the child. Services to birth parents are free of charge. Services are available anywhere in South Dakota, LSS can travel to meet with birth parents. ​Parenting and adoption options are explored: If parenting is the decision, the counselor will assist in setting up community resources. If adoption is the decision, the birth parent can choose from adoptive parent profiles, including pictures and information about the family. The birthparent also can choose what degree of openness they want in the adoption plan. LSS does provide both open and closed/confidential adoption. The purpose of this program is to assist birthparents in reaching a permanent plan for themselves and their child. Adoption Program provides options for those desiring to pursue having a family, including Infant Domestic Program and International Adoption Program. Training and Home Study Services are provided prior to placement of a child.

Medication disposal drop-boxes for expired, unused or unwanted controlled, non-controlled, and over the counter medications, including pet medications. Items that are NOT accepted include: trash, regulated medical waste, sharps, syringes, thermometers, hazardous waste, aerosols, illicit drugs, or commercial waste. Infant Sleep Safety services provide education on infant safe sleep practices and link families to resources when the purchase of a crib is a financial hardship. ​Immunizations services are available for children from birth through age 18 for all universally recommended vaccinations including influenza. The Vaccines for Children's Program provides vaccines for children who are uninsured, have insurance that does not cover vaccinations, are Medicaid eligible, or are Native American or Alaskan Native. For families with insurance coverage, the Department of Health is an in-network provider for Sanford, Avera/Dakotacare, and Blue Cross/Blue Shield. ​Pregnancy Support Services include regular visits with a nurse to share information about pregnancy, to provide assistance in obtaining early and on-going prenatal care from a qualified healthcare provider, and to link women to resources they may need to support a healthy pregnancy. Teaching topics may include: how your body is changing and how your baby is growing; eating healthy and gaining the right amount of weight; warning signs or symptoms that indicate you should contact your doctor; breastfeeding education and support; and caring for you and your baby after delivery. The Watertown and Sisseton locations are also able to provide car seats for all ages at no cost.
